Might want to post a screenshot, since this seems quite bizarre...
* When you say "more in my current playthrough than any of my previous ones" do you mean that you've always experienced this issue in some degree or other?
* Does it happen at the start of a gaming session, or only after a while?
* Have you considered the possibility that your video card is dying, or simply overheating?
* Are you ever able to reset it after the issue occurs, and if so, how do you do this?

We'll most definitely also want to see the temperatures being reported from your GPU thermal monitor during the worst of the artifacting.
See the Sticky that says "Before anything else", and follow that as your guide.

DA2 version: v1.03
CPU: Intel® Core2 Quad CPU Q6600 @ 2.40GHz (2389 MHz)
RAM: 3.24 GB
GPU: NVIDIA GeForce 8800 GT with 512mb VRAM and running the 275.33 drivers (I know they probably need updating...)
Sound Adapter: Speakers / Headphones (SigmaTel High Definition Audio CODEC) running 6.10.5511.0 drivers
OS: Vista Home Premium
DX: DirectX 11 (Jun2010) or later
Problem details:
Charater models are "deforming", almost as if single polygons are being rendered large distances away from the character but are still connected. (I'm going to be playing DA2 later so if it happens again I'll try and get a screenshot). Issue seems to be occuring mostly on the wounded coast. Issue occured in last playthrough (completely at random, no pattern) but saving and reloading always fixed it.
Started occuring during my second playthrough (this is the third playthrough on my PC I'll check with the person who did the second playthrough whether he was having the issue).
Problem usually occurs on the Wounded Coast and very occasionally in Dark Town. May have occured in other areas too though (always hard to tell, for some reason the areas in DA2 seem to blend together...)
Usually saving and reloading fixes the issue.
As far as I can recall the only mod I'm running is a texture replacement for the hair models (I'm pretty sure it's this one: http://www.dragonage...ile.php?id=2640

Very occasionally and I could always fix it by saving and reloading.

Kind of a mix, it's a bit random. At the moment I'm on the sword coast and it's happening almost straight away.

My card has always been noisy but the only game is ever really come close to overheating in is Crysis. With DA2 it doesn't even make a lot of noise. I'll run MSI when I'm playing later to get some GPU temp stats.
* Are you ever able to reset it after the issue occurs, and if so, how do you do this?
Like I said, reloading sometimes fixes it, but it seems a lot more persistant this playthrough.

That's not a "dangerus" number, unless it's at idle, just running 2D graphics, idle should not be over 45-50 Celcius, and any number reaching the mid-90s is getting into "look out" territory. In addition to dust in the GPU's own heat sink, look at the power supply for a build up in there, and on any of the case's fan blades, intake vents, etc.uberdowzen wrote...
I loaded up MSI Afterburner so that I could check my GPU temp while I'm playing DA2 and I noticed that it's currently running at 86 degrees Celsius. Would I be correct in saying that's far too hot? I sense my weekend disappearing...
The case itself should not be inside any kind of enclosed area, nor backed up close to a wall or other blocking surface behind the case, where it wants to vent its exhaust.
