Has anyone ever actually managed to successfully complete the quest and have it appear as such in the quest journal? It seems to only ever state the quest as failed, even if the king tells you he'll adopt the boy into his House. It's extremely irritating.

I was hoping that it didn't always occur - that perhaps certain circumstances might allow for the quest journal to update correctly.
No matter. I decided not to sleep with the noble hunter after all on this playthrough, so I won't be doing the quest regardless.

Yes, I have completed it.
It was a while back, so I don't recall exactly what I had to do.
I think it required going back to the mother of the boy and talking to her on a number of occasions to trigger the completion of the quest. You have to succeed at having her accepted into the royal family, I believe.

But -- as a previous poster noted -- there is a bug that makes the quest appears as failed for characters who did not even father a child with the female dwarf in question (Mardy, or whatever her name was)

Okay, so I reloaded that game and had a quick look at it.
The quest “Of Noble Birth” should log as complete by doing the following:
--you must play the male dwarf noble origin
--the character must father a child on Mardy during the origin part of the game (before being recruited by Duncan)
--when the character returns to Orzammer to do the dwarf treaty part of the game, he must talk to Mardy and reassure her he will do all he can to get her into the royal family
--in my playthrough I supported Harrowmont as king -- I’m not sure if this is essential to completing the quest, but that is how I played it (how it works with Bhelen I don’t know)
--as soon as Harrowmont is proclaimed king (right after completing Anvil of the Void) you ask him to accept Mardy into the royal family
--once you have Harrowmont’s permission, you go back to Mardy and tell her she is part of the royal family
The quest should now log as complete, but it doesn’t. There is a bug that causes it to log as failed. The bug also causes the quest to log as failed for all characters -- even the origin stories where the Warden wasn’t a dwarf and never had anything at all to do with Mardy and thus never had a child.
So ignore my earlier posts and please excuse my bad memory.
