But real sadness... I guess Overlord's ending comes close, but other than that, not really.

Still I think finding out Tali's dad was a traitor, dead and whatever was sad, as was noticing that both Mordin and Miranda (who I see as big players in ME3) died during final mission. Especially when I found out that no one would have died if I had like 3 more paragon points to convince Miranda to forgive me and Jack. But there are no second chances in real life and that's what I go with, butterfly effect happens.
I don't know if I'm older or what but I miss the feelings KOTORs gave me when Malak revealed who I am and when Kreia slaughtered Jedi order. Both were awful "NOOOOOOO!" moments that made me feel sick

This entire part of ME2 makes me cry incessantly: have played & seen it many times and I keep weeping like a child through the whole thing.

1. Salarian step-father discussing with his step-daughter what to buy her mother.
2. End of game where Shep places his/her hand on the coffin of a fallen comrade (if you lost one).
3. The Asari representative of the company doing unnecessary and invasive tests on the Feros colony people and you convince her to cease them (diplomatically)
4. Miranda's sister.
That's just a few examples of moments that brought tears to my eyes while following the Paragon path in ME 2, and I am sure BW has more in store for us in ME 3.

The entire conversation with Wrex on Virmire. Heck, any of the conversations with Wrex in the first game concerning the genophage. You can feel his anger and his frustration.
Leaving Kaidan/Ashley behind on Virmire.
Benezia's death scene. "Goodnight Little Wing, I will see you again at the Dawn. There's no light. They said there would be light." After all she suffered trying to do the right thing, at the end, her last bit of hope was snuffed out, and of course, how much of her estrangement from her daughter was Sovereign's fault?
Saren's last moments, if you Paragon/Renegade him into shooting himself.
The entire "I remember me" sidequest for Colonial Born Sheps.
Talking to Jack in her cell on Pragia.
Tali finding her father.
The scene where Mordin offers a prayer over the body of the dead female Krogan during his loyalty mission.
The end of Project overlord.
The end of Kasumi's loyalty mission.
The end of Samara's loyalty mission, and the conversation you have with her after "I am a broken vessel of sorry and regret." The transcript of the conversation she had with her other two daughters in her SB dossier.
Shepard's "Come back soon" after Liara leaves in LotSB. Liara's "You were gone..." breakdown if you try to take to her about your relationship right after you kill Vasir in LotSB.

Hell yes. These all get me to some extent or another.
I know I keep quoting lists here but I can't help it. All of them (and most of the posts in-between), I totally agree.
Saren's suicide is one of the absolute best for emotional impact, I think. I have this crazy headcanon thing going on, even, where many of the lyrics to the ending theme in ME1 are tied to the strange relationship between Shepard and the tucked-away decency in Saren as seen on Virmire and at the Citadel if you Paragon him.

You probably get this much too often for anyone's health and I hate to add to the broken record here but... you should totally try to fix that and play ME1 ASAP.

Plenty of poignant ones, but hard to say which ones actually made me genuinely sad. Ilos possibly, something about driving past those pods in particular really made the tragic fate of the last Protheans hit home. The music helped.
Virmire was definitely a powerful moment, and I concur with Jeff regarding Saren. He isn't quite Loghain, but the tragedy present in his story definitely makes him a more memorable antagonist.
I would say that the suicide mission has the potential to be incredibly sad, pretty much from beginning to end. But that all depends on the playthrough, and it loses much of it's power on future playthroughs, especially if you have ended up metagaming somewhat because otherwise noone would ever die.
Mordin's quest deserves a mention as well I think, Mordin is so much more than comic relief and it really showed there. Really hoping for a powerful payoff to the genophage plot in ME3.

Lost Odyssey is the only game that's ever made me feel uncomfortable, as one of the characters in it is a dead ringer for me, shares my name and many of my personality traits. Yeah, that was... unpleasant.
Only one game has ever brought me to tears. Silent Hill 3. Harry.
Concerning Mass Effect, no. Nothing in it made me "sad"... I was a little worried during the suicide mission, thinking it was going to be a lot harder than it was. Virmire never bothered me at all - emotionally at least. Kaidan and Ashley were my go-to team.
