Kind of what they had with Parasite Eve: Third Birthday, only that it isn't "exclusively" fanservice.
Say that a player goes into a room and gets jumped by a group of shrieks. As the player takes damage, their armor equipment gets damaged and it shows after the battle's over. If the player keeps wearing that same armor, then more damage even after healing will only make the armor look more worn and torn. Eventually, the player will either have to equip a different set of armor or be stuck wearing battle scared equipment that only offers a tenth of protection.
Then after the dungeon/quest is over, you can either go to homebase or an armorer to have the armor repaired/upgraded after a certain amount of time.
The damage progression would also be different with each class. Warriors are frontline tanks, so they're armor won't show damage as easily; Mages get ripped apart in melee so they'd have their robes or whatever shredded faster and Rogues would be in the middle since they're not as durable as warriors, but quick enough to avoid attacks.
You could even have NPC's comment on how messed up you look and either try to help you or jump you believing that you're too exhausted to put up a fight.
Is their potential for pointless fanservice? yes, but it's nothing that would distract from anything we've already seen. It's not like the feature would be like "Two Girls, One Cup". Would the Media Watchdogs have a problem with a female rogue fighting darkspawn while semi-nude from armor damage? yes, but when have we cared?
Dragon Age is already an M-rated series featuring sex scenes, language, raunchy dialogue, violence, blood, gore, people exploding into chunks when you kill them, murder knives through the head, ogres crushing people into bloody pulps and did I mention all the blood? So would adding clothing/armor damage really be too difficult to imagine?

It sounds... nice, but an extremely low priority for me. I'd rather they figured out how to put horses in first.

Interesting but I'd rather see other stuff worked on first. This seems more of a visual adative than an actual improvement. Though the way it sounds it would help make armor type mean more.

Violence, blood, disturbing scenes, murder, drugs, peril, grotesque physical features are all okay but a little skin flares up a warning bell.
Plus, as I said, it would a bit of a realistic touch since even strong armor wears down and needs to be replaced/repaired after enough battles. As for potential fanservice...we already have Isabela and the series is already M-rated, so what are we losing?
