I just got into TOB, and asked Cespenar to make what he could. I got the helmet out of WK and the upgraded unlimited bolt case/quiver, and those are fine. But I also got the Flail of Ages upgraded with the poison head, and it's... gone. It's not on the ground, I've checked my inventory several times (Bag of Holding included), and it's not in the boxes where I stashed leftover items.
Is this a known issue? I can hack it back in if need be, but I mainly want to know if this is liable to happen later and if I should be keeping track to make sure everything gets made properly.

Never had it happen to me, with the FoA or anything else. But being a very cynical person I've always used the second choice ("...just don't destroy anything...") in ordering the Imp, rather than the first choice ("...show me what you can do..."). Cespenar, being an Imp, may be playing a practical joke on you, and either destroyed or made invisible FoA. Is there any new dialogue if you poke him again? Does casting True Sight reveal anything when you next push the Tab button? Do you have a recent save so you could run thru the scenario again using #2 instead of #1?

It should just be a one-off bug. I have never seen anything like it.

I'd keep track myself. Can you reload and ask him to make it again? If it turns up it may have been once-off, if not there's something screwy.
If you don't have a save, the code for the +4 poison version is BLUN30C, and the full +5 one is BLUN30. You can add it with shadowkeeper, or CLUAConsole : CreateItem ("BLUN30C") (without the spaces).

This happened to me once, when upgrading the circlet of Netheril it just vanished.
I'm not sure, but I suspect Cespenar's script (which calls for him to fly around the pocket plane and talk about shiny ones) can interrupt his action to give the completed item to you.

When using editors like DLTCEP or Near Infinity, in the game files Cespenar is BotSmith, so his script is BotSmith.bcs and his dialog would be BotSmith.dlg.

I had something similar happen to me. I had two bags of holding with distinctly different items in each. I didn't look in either of them for quite some time and then needed a key that was in one and that the game was telling me I did not have. I looked in the bags and found that all the contents of one had been replaced by the contents of the other so that I had two of everything from one, but nothing that had originally been in the other bag.
While it was nice to have two Crom Faeyrs and other magical items that I'd put in the one bag, it was quite annoying to lose everything that I'd had in the other. I used Shadowkeeper to replace the stuff I could remember and just lost the rest. As it had been quite a while since I looked in either bag, I'm not sure when this happened or why. Sometime in the middle of Shadows of Amn, I think.

Did both Bags have the same item code? ("Bag31"). ShadowKeeper will tell you that when you open a save game.
It happens with potion bags too, if two or more bags have the same item code. Each container is a unique item, with its own item code. I can only guess that the contents of each container is stored in the save game file, listed by item code. I don't know exactly why it happens but it does.
