i used to watch my brother play Half Life 2, that scared the crap out of me. I think I was about 12 at the time.

Dead Space Extraction was pretty freaky.
Also the IFF mission in ME2, all those husks scared me. Every play through I spend the whole mission anxious and twitchy.

Amnesia of course... I tried it. It was scary, but not something I want to play. I want to have at least some methods to fight back.
As horror, it was excellent, but as a game, no thank you.

I don't know that any game has necessarily scared me so much as given me that creepy uncomfortable feeling down my spine - particularly American McGee's Alice.
Parts of Thief were disturbing (the attic in the Cradle). And I will admit to jumping a bit when playing DA:O and DA2 and the spiders suddenly appeared (spider-drop sound effects via headphones is a bad thing).

Amnesia top of list, played it alone for first 2 nights, then waited my brother to come to visit me for weekend, played that night with him.
And then we waited our uncle to come so we could play by him

ALOT of games made me scream doh geezus, Dead Space 1 and 2, sometimes even in DA when spiders spawn it makes me jump.

wow, a lot of people were scared by FEAR.I really didn't find it scary, FEAR 2 was scarier but not much. FEAR 3 only had Alma in it about two times.
Penumbra: Overture. I love this game simply because it can make me so paranoid that a dog is chasing me when its not.
Resident Evil: Outbreak. I was young when I played it and I started pissing myself because the zombies were closing in and I couldn't find the to get out. Turns out one of the survivors had it. Still the best in the series.
Dragon Age 2 - Giant spiders everywhere plus an even bigger spider as a boss fight. I had to sit in the corner and let my party members deal with it.

Hellbound reminded me of something I forgot to add in. Mother 3 has an especially unnerving scene that's similar to Magicant. You ate some suspicious mushrooms, and the affects are significant. In the case someone plans to play the game, I won't spoil anything, but I can say it's quite disturbing.

Since I have the survival instincts of an angry, undead honey badger, nothing really scares me in games these days.
I do remember getting slightly creeped out by the redead things in LoZ when I was younger, though.

I'm a big chicken, and I generally don't play scary games. However, I've made a couple of mistakes/attempts in the past with the following:
Dead Space - I tried the demo (the DEMO!), and couldn't even finish it. Too scary.
Condemned: Bloodshot - I played for a while, but with my husband sitting next to me. I was still scared and stopped playing after a couple of days.
Silent Hill: Homecoming - too scary; I couldn't do it.
Strangely enough, I love zombie games, and haven't had much of a problem playing them: Left 4 Dead 1 and 2, Dead Rising, Dead Island, etc.
One that was a little scary, but I kept playing was Resident Evil 5. I played through the co-op with my husband twice, and once with my best friend. I even played it by myself. Boo on those who didn't like Sheva. Once I had her decked out in her clubbin gear, she was unstoppable.
Edit: Somehow I missed Demon's Souls and Dark Souls. Both are terrifying, but since you die so often, it almost becomes less scary.
