If you want to find superior armor or weapon, there are none. In this game, armor and weapon superiority is depend on tier. Bonus stats on armor and weapon is nothing actually. For example, Top Sider Sword is Dragon Bone tier sword with +6 damage against undead, against anyone else it is just a Dragon Bone tier sword. Drake/dragonskin fire resistance only good against dragon and some fireball mage, if you are an auto attacker the stamina regen is insignificant (only good for Arcane Warrior), against anyone else it just an ordinary armor.
So, just seeking higher tier ordinary armor and weapon is just fine. You play as Rogue anyway, you have poison, traps, bombs and stealth, you only need to set up strategy with your party members. We can get bonus stas, resistence and regen from items, salves, and party member buff.

Sadly I haven't fully played as a rogue (I'm mainly a mage).

The only skill that is really necessary is Combat Tactics as a prerequisite for talents. Coercion is handy (and fun) if you want to play a certain way; and it's the one skill that only you get. All of the others don't really matter; you can barrel your way through the game without making a single potion or poison, or setting a single trap.
To examine your situation, you need to clarify your terminology. For example, armor has an Armor rating, which is separate from Defense; some have a Defense bonus. Weapons have a Damage rating, and may have an Attack bonus, but Attack is based on Strength and Dexterity. Armor and Weapons are graded by Tier, from 1 to 7 in the main game.
There are different kinds of leather: "Inscribed Leather" is Tier 6, which is pretty good. Each kind of weapon, like "Longsword", does the same damage at a given tier. Dwyn's Sword is Tier 3: not so good. But it's the bonuses that matter. A Tier 3 with good bonuses is better than a plain Tier 4, or maybe even a Tier 5 or 6.
Finally, you must have improved the right attributes when leveling up, and basically ignoring the ones that don't matter for your class. Those base attributes are used to compute the stuff that really matters, like your Attack, Defense, Damage, and Spellpower; upon which your bonuses (from both items and talents) stack, and which govern the game mechanics. (That's especially true for the one-on-one fights; to work well as a team without you having to micromanage everything, you need good Tactics.)
To compare, I have a late-ish-in-the-game level 19 dual-weapon Rogue. It's not an extraordinary build, but benefits from me having played the game a few times through already. Some items/stats are:
Light armor Tier 7, Armor 9, +9 Defense, +4 Armor (so 13 Armor for this chest piece)
Longsword Tier 6, Damage 10.5, +6 Attack, +2 Damage
Waraxe Tier 6, Damage 9
Armor 21
Attack 112
Defense 118
Damage 35.7/34.3
I also have a helmet, gloves, and boots. The two weapons have two rune slots each, both filled, and some other bonuses I did not mention. I found everything except the chest piece, which I bought, one of those really good items. The rest of the party has some Tier 6 and Tier 7 items with good bonuses that were also just found.

I think this is why I'm having so much difficulty with the game. It's very late in the game for me right now (right before the last mission). I've had to play much of the game on Easy mode and I still found some parts difficult (even on easy). I suppose this is because I haven't paid much attention to getting good equipment and using skills like potion making and trap setting. In a particular duel late in the game (I won't mention which one so as not to spoil it, but people who have gone through this part will know what I'm talking about), the only way I could beat it with any of the characters I had on hand was by running away until dirty fighting recharged (I was using a rogue Warden with coup de grace). By the way, this was on EASY mode. My rogue still has leather equipment. His chest armor has a defense of between 7 and 8. His weapon has an attack of 10.5 and is not enchanted as I only got it recently. Up until that point was using Dwyn's sword (8.4 attack) after enchanting it. How can I get sufficiently strong weapons and armor? I've checked the merchants in the game but none of them really have anything more than what you could find from killing enemies.
I play as a Rogue, but I find myself not using potions or traps either. Mainly because I don't have much trouble with the battles. I play on normal difficulty. I do not use Tactics. I micro-manage every character, every battle, every micro-second.
My armor is slightly better than yours, I believe the armor rating is 9. What I'm wearing I've managed to pick up from fallen enemies. Maybe a few things I've bought. I have access to the vendor at Warden's Keep. He has some nice things.
I duel-wield daggers and my main dagger has 3 rune slots, my off-hand 2 slots. These are filled with runes I've found or bought from the vendor at camp. From what I understand, these daggers are not the best in the game.
I have Momentum talent which is on all the time. If I get behind someone, I put a serious hurting on them. I use Alistair as tank and he wields the Starfang sword. Morrigan is always in my party and she is a devastating weapon with Sleep and Cone of Cold. Wynne keeps everyone healed.
My total armor rating is 15
Defense is 91
Attack is 85
Damage is 20.5/20.5
I am nowhere near finished the game. I am level 15.
Maybe you should look at the vendors more closely in Denerim. Don't know what else to tell ya.
