Nothing except if people fail to get to your standards and fail their mission for one reason or the other. If they fail fulfilling a role that was outside their choice they have to die did they not?
And still I don't see oficial word proclaiming anything...so no "sauce" yet. If it has been states in a codex or something, could you link me to it in the Dragon Age Wiki (where I believe all the codexes are), please?

You're right in that they aren't executed. If a Qunari fails a task they are re-assigned into an area where they can still be productive. Tallis is the best instance of this that comes to mind. She was assigned menial labour after failing a mission, until such a time as the Qunari decided she might have use.

Bas-maker, maybe. I doubt they'd have a name referring to that condition. Names usually refer to someone's role, not their state in that role. She is free to act in her role however she wishes, lol.
