I'm on my 3rd playthrough of ME2 and have started the loyalty missions post-collector ship. For some reason, I still haven't gotten the 'dinner with Kelly' dialogue. On a previous playthrough, I got it one mission after Horizon. I know I've always taken the paragon options with her... what gives?
The only major difference I can think of is that I did LotSB after getting all the squad.
... I'd like to be able to run around the galaxy without having to remember to feed my fish. Any ideas?

Well there's your problem, you did Lair pre-endgame. 
But seriously, if you say you didn't do anything different, it should have triggered. If not, no idea; it'd be silly if Lair was the reason. Did you romance Liara? Or anyone else previously?

My understanding is that the dinner is triggered by conversing about the squadmates always taking the flirty option. If you have all conversations it seems to trigger off 2 conversations after Horizon or always for me, after recruiting Samara.
If you haven't gotten it go to your cabin and back to the CIC for another convo with Kelly until you exhausted all options.

Its after you recruit Samara, and say to Kelly "Didnt recruit Samara for her looks, we've got you".

There are (at least) two ways to trigger the dinner. One is to compliment her appearance after recruiting Samara. The other is to ask for a hug after the Collector Ship.
Both require that certain prerequisites are met. For instance you need to be on the romance track ("I might do more than catch you") and I think you need to have investigated all her dialogue (asked her about her role on the ship and so forth). I would assume you need to have had the "Character matters not race or gender"-talk (can be had after recruiting Garrus, Thane or Tali).
If you do not meet all the requirements you will simply get a hug when asking for one and Kelly will simply say you are sweet when you tell her she is beautiful. This is also what happens if you trigger the dinner one way and then use the second trigger later (seeing as how the dinner can only happen ones). If either of those things happen then you should ask yourself what you have missed. If you do not even have the option to compliment her (romance) or ask for a hug (friendship/romance) you are not on the right path at all.

Yeah I don't see how it would, but I never did LOTSB before I had dinner with Kelly so I don't have any direct experience to add.

It varies depending on how many missions you do and in which order, as well as how often you talked to her in between each and whether or not you were flirty at every opportunity. If, for example, you skipped over one after-mission conversation, the dinner will trigger one mission later than it normally would. As best I can tell, the one constant is that it always triggers due to picking a flirty response, and it can potentially be during any post-Horizon conversation which has one.

No, you can be on the friend track too. She even has a slightly different response to the "just friends" invitation.

Hmm... I did get a hug from her after the collector ship mission, and I'm pretty sure I've flirted with her each time. I honestly don't remember what I said after Samara's recruitment... I'll keep pressing and see what happens. I'd hate to have to re-play 5-6 missions though

I don't think telling her "I want to do more than just catch you" is that essential (I didn't once and still got the offer). Anyways, you can choose that dialogue option several times and pick different responses.
Agree with her views on sex (gender doesn't matter, bad boys are sexy (you can either agree with her as femshep or point out you fit this category as male, methinks). Complimenting her after recruiting Samara I don-t know about - I always got Samara after CST for some reason and by the end of this mission (CST I mean) i usually have all the above done, she is all "I was worried about you" and then in a few flirty options - caboom! dinner.
I am absolutely positive that during that one playthrough when I got the friendly hug (didn't flirt with her) I never got to ask her upstairs.

I was wondering the same after getting Samara and not getting her in the room, so I reverted to last save and went through her dialog, I noticed I missed the one about her sister caring for animals, now I gotta go back and do the mission again, second time's a charm.
Liara shouldn't affect it, just the shipmate romance will, be sure to flirt after every mission, that should take care of it and have you ready, I plan to have several endings with different romances since I saw the 3rd games dialog, it's very hopeful I might add.

I got the hug after the collectors, that was it, I'm a good friend, if nothing happens with Samara I guess it's a fail, I'll not play this freakin thing again for just her!
