So, my huge dilemma here is as follows:

Approval Bug seems to be caused by negative approval before you recruit someone. From what I can tell though, you apparently get -10 Approval with Justice if you recruit Velenna into the Grey Wardens or during her quest or something. Will this cause the Approval bug or this only caused by side quests specifically and not main quests (as it seems some quests effect approval overall regardless if they're in party or not and some only seem to trigger if they're in the party.)

What about Steafan, if I free him will that net me negative approval with anybody? The wikia is relatively complete on most of this, but there are a few quests/walkthroughs that don't denote positive or negative approvals. I'm so damn stressed out trying to manuever my way through the game without having to play it a thousand times. Any advice is helpful.

Also, not exactly the bug, but... is there an "optimal guide" to get Oghren's quest if I have the import from DA:O bug, or do I just gotta hope for the best?

Not too sure what you mean by the 'approval bug'. I always get Justice last for gameplay reasons and I've never noticed his approval bug out.
But Oghren's approval is a pain in the ass. Basically Awakenings remembers the gifts you gave him in DA:O, even though it resets his approval of you. So if you gave him lots of gifts in DA:O, the first personal gift you give him in DA:A will net you only +6 rather than +10.
Obviously it would help if you didn't give him all his gifts from DA:O or still have some of his gifts imported. But even if you haven't, it is still possible to get Oghren's quest (just). Give him all his personal gifts and aim to hit all his location dialogues for maximum approval (http://dragonage.wikia.com/wiki/Oghren). If you miss one or two, you still get some approval for giving him other people's gifts. You need 75 to trigger the quest.

I think the "approval bug" is where it automatically goes to 100 and sometimes is listed at "Love"

I easily maxed out Oghrin's Approval on an imported save, I even went so far as to remove him from the party when I recruited Velanna and then ran back and put him back in the part. Also, the negative approval Justice gets for doing the Joining with Velanna only applies if he's in the party/recruited.

My only problem with my game, amazingly, is trying to trigger Velanna's Exile.
