From confirmed? Definitely Kaidan for powers - especially I wait for someone with Warp and...

Hm. Logical choice for gameplay would be Liara (crowd control, Warp bombs), but I can't stand her. Vega looks okay for now, but I usually don't take tanks; Tali is useful against synthetics and not so against organics, so was Garrus - he's a sniper, though, and may be taken for that. I'd pick damage dealer to squad with caster, but Virmire eliminated this possibility.

So who would I pick? Kaidan and Garrus. Who would I rather pick? Kaidan and Miranda or Kaidan and Jack or Jack and Miranda.

Definitely Kaidan, since he's Shep's LI. Who gets the second slot depends on who comes back and what's going on. But I tend to pick my parties based on personality, not so much on abilities. Of the confirmed, it'd probably be Garrus. I think it'd be fun to have a Kaidan/Miranda party too, but I'm all about awkward tension for story purposes.

There can be no dream team with only the announced permanent team members.
There can be no dream team without Miranda.

I'd love to continue with my dream team of ME2, Miranda and Jacob, but Miranda and any other human with biotics might work just as well. Miranda and Kaidan seems a good one, provided they give Kaidan Pull.

Ashley and Garrus. They're my favourites out of the annoucned squadmates, and their abilities complement my adeptShep.

Though depending on who else is announced, temporary companions, etc, Garrus might take a back seat every so often.

I'll stick with original squad combinations. Kaidan, Liara, Garrus and Ashley. So far there's nothing that makes me like Vega and I somehow doubt that ME2 characters have much role in ME3. Tali and Wrex are question marks, if they are available then I guess that I would use them too.

Soldier Sheps have less need for a Tank, but its fun to have a Shep Wrex Ash/Shep Zaeed Grunt painfest march on a fortified position. Otherwise, I try to balance a biotic and a tech expert as the mission demands. Occasionally two techs vs. the Geth with Shep tanking. I'll use two biotics rarely, and then only when I'm best served by getting the small fries out of the way for Shep to steamroll.

Vanguard Sheps take two biotics often, because tag team powers are fun. Tech as needed and a Tank for tough firefights. Someone needs to be the bullet spunge, don't complain Grunt, like your role.

Infiltrator Shep needs distractions for "Sniping the targets" action. Whatever form they need to take for that mission they do. Biotics for rending shields, techs for overheating weapons, tanks for absorbing bullets.

This. The reason you have so many squadmates is so you can min/max out the mission. You're not going to attack synthetics with two biotics and no tech support, that's just dumb. If you had a bunch of exotic cars, would you stick to just one? No, you drive a different one everyday, and depending on the weather, you have to pull out your '94 land rover. Sure, it's not the prettiest but it's lovable and it gets the job done damnit!

for my 2 main sheps its team dextro all the way, my fem shep is gonna park her former flame on the normandy to handle the paperwork and have Garrus join her in the fun times groundside. Male shep has his brother from a turian mother with him and LI Tali (to help my soldier shep deal with shields lol)
