What happened to the difficulty system. What if I want to have a good RPG experience with HARD combat? Of course, they have to replace everything that was good about the past 2 games with something stupid.
Still there. There's screens of the option menu where you can change difficulty, helmet toggle, choice preference and basically make your custom mode. The main menu mode options are basically prefabs.

It's BREEEEELIANT!!
Oh, and has anyone mentioned the dev resources required for such an option? Bioware can't afford to cater to everyone.
The resources argument doesn't really make sense to me. Dialogue options are usually handled by scripts and are usually the easier scripts to make in moddable games (like NWN series etc.). Something like this may happen script wise during Mass Effect convos (not using script language and summarizing):
Script Name: Manual_Convo_123
-on player click of NPC, begin conversation
-play dialogue, open dialogue wheel
-on dialogue end, show options a and b
-on click of option a or b, play dialogue for respective choices
-on dialogue end, show options c, d and e
-on click of options c or d, play respective dialogue and activate quest marker 1
-on click of option e play dialogue and activate quest marker 2
-end conversation
The auto-choice dialogue could replace most/all dialogue wheel convos by calling up their script IDs and replacing them with auto dialogue scripts like this:
Script Name: Auto_Convo_123
-on player click of NPC, begin conversation
-on player click of NPC play dialogue, hide dialogue wheel or not call it up at all
-on dialogue end, choose a, play dialogue
-on dialogue end, choose c, play dialogue
-on dialogue end, acitivate quest marker 1
-end conversation
Not exactly a tall order in resources/programming if it works something like this.
Edit:
Or it could be all one script and they use "if" conditions around the possible choice modes.
