There is a quest (the last one at the bottom) in Dragon Age: Journeys named 'Toil And Trouble'. Do I need to play through that quest to count as I have completed the game to unlock the Achievements? And how do I start the 'Toil And Trouble' questline?

It's not needed for any of the achievements or items. If you still want to do it, you start it by asking the potion vendor in orzammar near the exit if she can make any special potions, and she sends you to get mushroom stalks. You can get these by using those mushroom patches in the deep roads.

There's an NPC tucked away near the entrance of the city. He's a merchant that sells only potions. He gives you the quest to gather mushrooms for a special potion that permanently increases your stats.

What bothers me is that after I start the quest to get the Mushrooms, the Toil and Trouble quest on the sidebar is not lit up like the other quests when you unlock them. Is that a bug?

You have to finish the quest to "unlock" it on the left.

Not to derail your thread but is there more help for this? I'm playing on hard and I have no idea how i'm going to beat an ogre with my party at full health....

Very liberal use of mind blast can get you through just about anything, works even better if you're a mage with it and get the spirit healer to get it too. Just be sure to have mana pots on hand.

So two mages is generally the 'better' way to get through the game? i.e. your party is 2 mages and 1 grey warden?

That's probably the fastest and easiest way, yes. Just have to be careful of the more powerful ranged attacks like the genlock alpha's rock toss.

Take heaps of healing and lyrium potions with you for the fight - they cost next to nothing and make earning the achievement so much easier.
During the Ogre battles, if other enemies are accompanying the Ogre, concentrate on the other enemies first so that you kill them, and then put all your effort into the Ogre. Position your warriors (depending on party composition) behind and to the side of the Ogre so you get the flanking and backstab bonus and use your special talents as much as stamina allows.
When the Ogre is nearly dead, use your turn to use the health potions to top up all your party members health, and check their character pages to make sure they are all at full health. When everyone is at full health you can then deliver the final blow to the Ogre, and earn the achievement.

I just beat the Ogre at full health on Normal. Is the Hard setting really as difficult as it sounds? Do we get more exp per fight and get stronger equipment on Hard setting?

You dont have to be at max hp when you do the killing blow on the ogre. You only have to be at max hp at the end of the battle.
I always killed the ogre first and took my time healing the party up. And I never used potions

Was that on Easy setting? The Ogre is always accompanied by 2 minions. If you do not put down the easier to kill minions first, you will have to keep those minions off you while you hit the Ogre. Sounds harder to me.

Get the Bard, use Distraction on the Ogre (100% Hit), and whack the others, then proceed to whack the Ogre at will, should you need a break to refill mana/sta/health, use Distraction again. Pinning Shot (slow/archer), works also, he does minor dmg in ranged combat.
Im whacking the 2 Glenlock Alphas and 1 Ogre in Hard Mode without any problems what so ever...

Ive done pretty good and have been able to keep my party deaths to a minimum. However the last (?) fight between the two alphas and the ogre I just can't win. I'm not sure why. I've tried teh above tactics but that ogre got critical hits 3 times in a row! I'm just not sure how to stand up to that.
I can't repel firepower of that magnitude...

Finally got it. All it took was the AI not landing so many unhealable criticals and I was able to triumph!
