For reasons I won't go into detail, I haven't finished DAII the 3 times I've tried (it isn't because I don't like it, far from it).
I last played when the Item Pack 1 was released or was close to release.
Are the DLC item packs worth it?
Are the last two Story DLC worth it?
Have there been patch fixes? Has a class been nerfed or buffed?
So...what have I missed?

I can't speak to the Item Packs as they don't interest me.
The story DLCs, which I've purchased but haven't played yet, have generally gotten excellent reviews.
The Reaver specialization for Warriors got nerfed a little, but the class is still playable.

I find the lore behind the item packs interesting, and generally the DLCs are great. Legacy I found to be a great DLC.
MotA has a horrible moment for Hawke if you're anti-Qunari at the end. Play as anything else though and it's a great -- albeit flawed -- DLC.

Depends on playstyle. At least 2 hours for Legacy if you do everything and read everything, and at least 2.5 hours for MotA.
But other people -- myself included -- have had them go on longer than that.

That depends on your playstyle, but I've heard that Legacy takes 1-3 hours to finish and Mark of the Assassin runs for 2-4 hours.
