As the title says do the companions keep the equipment I have on them when the Acts change. Also what happens to Carver's equipment if I send him to Gray Wardens (if I lose it when exactly should I remove it so I can keep some of it)?

yes they keep their equipment.
not so sure about your sibling; i'm pretty sure that either their equipment gets returned to you (or it just gets lost), but i can't really remember and my brain is a little bit dead atm.
safest bet is to just remove whatever they're wearing.

Do you know at what point in the quest I would need to remove my sibling's equipment, I need them to have it for while they are fighting with me but don't want to get stuck in a cutscene and then have them unselectable before I can remove it.

For warden sibling you can remove it after the ARW-fight, for gallows sibling, before you head into the deep roads.

I am not sure it is necessary though. I think they at least keeps their assocories. (Not sure about weapon.)

They lose any weapons you give them. If you want them to stay equipped with something decent, you can give them the sorta meh DLC gear you have available since they are not worth anything and by time you get to the end of Act 1, if you metagame out your quests and all that good stuff, will start start degrading.
Accessories will remain on the sibling character, so if you give them their special accessories from the DLC packs they will still have them.
