No idea for a movie intro though, they generally don't have the "trailer for the entire thing" intro at the beginning.

A few of note, for me:

Cartoon (anime-based, at least): Robotech: The Macross Saga

OVA anime: Gunsmith Cats

TV: Wonder Woman - why? Lynda Carter. And comic books. (I was an impressionable child)

Movie: Star Wars: Episode 4 - this doesn't have the whole bit with the 20th Century Fox drumroll and such, but it's the entire package that makes me misty-eyed. See above note about being impressionable.

I can't immediately think of games that I love the opening sequence of (mainly because I usually click past the intro in my enthusiasm). But another movie of note would be Xanadu. Horrible movie in total, but with great music. The intro (which I could not find) was the Universal Pictures logo with various planes and air vessels spinning around it in a chronological parade of science, and then it transitioned (shortly afterwards) into ELO's The Fall.

Oh, and Disney's Beauty and the Beast was awesome too.

Game intro: Mirrors Edge. Just loved this it's short and sweet and to the point. It sets the mood for the rest of the game.

TV Show intro: X Men Cartoon. I was pretty young when i first saw this and i could barely understand the complicated storylines but the intro blew me away and made me an X Men fan. It just says "My intro kicks ass. If you do not watch me you suck"

Anime Opening: AIR. This opening is just beautiful and really sets the mood for the show it's a relaxing and beautiful show. But a very sad and tragic story so if you watch this bring tissues!
