Nice chart, BatmanPWNs. I always try to use every team member in some role or other in the final sequence. I have to include fighting the Occulus so that I can include Zaeed. It's not necessarily a perfect strategy but I get a sense of satisfaction for giving them all a reason to come along.
Occulus: Zaeed and either Garrus or Grunt though, tbh, that fight is usually over seconds after I release the trigger on the portable nuclear bomb.
Vents:
Fire team: Jacob
Tech specialist: Tali or Kasumi - Tali this last time. Whoever gets chosen here, the other comes with me at the end.
Squad for this segment: Thane and Legion
Long Walk:
Fire Team: Garrus
Biotic Specialist: Samara
Squad for this segment: Jack and Grunt (though Grunt is not ideal here)
Escort: Mordin
Final Fight:
Squad: Miranda and either Tali or Kasumi

Who did you put second in command to lead the fire team? Garrus, both times.
Who did you send to go through the vents? Depends on whim. Either Tali or Legion. Most often, it's Legion.
Who did you choose to escort the survivors back to ship? Any of the squadmates (besides Jack) with the lowest defense scores, so either Kasumi, Tali, or Mordin. I tend to pick Mordin more often.
Who did you choose to help you through the seeker swarm? Jack. I like the symmetry: that she has to trust us to protect her from the Collectors and husks while she's essentially helpless, while we must trust her to protect us from the seekers.
Who did you take into the final battle against the human reaper? Thane and Miranda. In fact, that's who's in my squad all the way through the Collector base: Thane, like Zaeed, has that massive damage bonus, and Miranda adds in some more with her passive. Also, both have Warp, and it's barriers and armor throughout. When I'm not Charging, I can throw off a quick Pull for Warp detonations. That or have Thane use a Throw or Miranda use a Slam. I also like to have Miranda chime in during the chat with TIM.

Oh, played it through many times, right now I have erased all my saves and re-doing four for import.
In the one i just completed, which are the first of the four i started out by using Garrus as a 2nd team leader and Tali through the vents, i brought Grunt and Legion with me.
Samara held the bubble while Miranda was the 2nd, 2nd fire team leader, i brought Mordin and Thane for that bit.
Jack escorted the Survivors.
In the last battle i brought Thane and Miranda.
No one died.

This is a very stupid question to say (as I have beaten ME2 2 times) but, do you need to have all of the shield upgrades for the crew (Im not talking about the ship shields). I sorta forgot how I saved all of my crew on my other playthough. On my current playthough, the next upgrade for the shields costs like 175,000 and I only have like 35,000, and Im right as before starting the Reaper IFF mission. As of right now, everything else is all good (everyone loyal and ship upgrades). Am I all good for perfect SM run?

Grunt for his toughness, Garrus for his Overload ability
Deaths of Active Teammates: Zero.
Everybody got back alive. The only crewman saved was the good Dr. Chakwas. Poor Kelly was turned into a Reaper Slushie.

I just finished what I hoped to be my final playthrough of me2 for me3 and I chose my team the same way as last time when everyone survived but now I have a dead salarian on my ship, is this the famous bug and if I restart the mission is it possile for everyone to survive?

First time: Garrus (in-game reason: Shep was confident in him, and wanted Garrus to definitively snap out of his "I failed as a leader" funk)
Second time: Miranda (in-game reason: Shep wanted to let her know he trusted her with the task, despite not giving it to Garrus the first time around)

Ms. vas Normandy. (in-game reason: Shep is confident in her tech. abilities, and knows nothing can stop him if he thinks she's in danger, them being together and all).
Brought Legion and Jack with him for this leg.
(in-game reason: wanted to keep Jack away from Miranda in case their infighting screwed up the mission, and Legion was not trusted either, having just joined the team- Shep was confident they'd work well with him though.
