Where is is located? I have found the other 3 pieces in the forest, but i can not find the gravestone for the chest piece...

West Brecillian Forest. Go to the tombstone, disturb it, and take it off the Revenant's corpse after you kill it.

Incorrect. The breastplate can be found in the Elven nruins. Complete the ritual and loot the main coffin thing.
