Why has customization of party members' armour/appearance fallen out of style?

Cosplay.

SWTOR has it, but an mmorpg is more focused on hard-core RPG people rather than trying to bring in a bigger audience from other demographics.

Ashley apparently has one casual outfit and another combat outfit.

Yeah it would be kind of silly if James Vega walked into battle rocking a t-shirt and some cargo pants...

Jack waltzed into battle topless and with baggy pants.

Why would everyone wear heavy armor again?

Because some characters are not trained or used to wearing heavy armour perhaps?
If you can move or operate efficiently during a mission, you risk becoming a serious liability for the team.
Thats why some characters might have lighter armor and protection when required, but still armour and protection of course.

This. I'd be outright pissed if Thane was forced to wear human light armor.

Armor fits on everyone. EVERYONE.

Tell that to the guy with the joined middle and ring fingers.

Or to the thief who specializes in infiltration.

Dumbing down, streamlining, saving time and resources. Thats why customization is gone.

You forgot to say something about CoD and/or GoW fans, Michael Bay, and EA.

I have heard that every 50.000 year EA invades from dark space to harvest a developer studio.

Oh, yeah: and something about not being a real RPG. Forgot about that one.

Something about 'iconic appearances' and cosplaying. I'm not a fan of it myself.

I don't mind that approach personally but Mass Effect hasn't really implemented it very well. I thought Morrigan had a very iconic look that didn't seem out of place. Miranda has her "iconic latex", it is certainly easy to notice too, but does it make much sense or seem in place? To me it doesn't personally.

Sure, as long as you don't actually think about it too much.

Morrigan is an interesting example. Morrigan has an iconic look BECAUSE her clothes/rag usually only change once (when you get her new rags from Flemeth's hut). The thing is, her clothes don't make sense either especially when in Denerim when you are supposedly trying to keep a low profile..

None of the other companions (the warden included for that matter) have an iconic look to them and while it gets mocked, brand awareness/recognition  _IS_ big business.

Nobody knows who the hell Morrigan is. The only people of interest are Alistair and the Warden. Why would she need to lay low? The Templars don't seem to notice her unless you bring it up. She is probably most likely pegged as "Chasined" which is odd to see in Denerim, but nothing to raise hell about or draw attention.

I never said they did. I was just saying Morrigan had a well designed unique outfit that made her "iconic".

You can change her appereance at any time. Her rags aren't even ideal pieces of gear for her if you're into stats. However I'd argue many keep her in her normal look because it is iconic and fits her and the setting.

Actually not thinking is the key. Miranda using her perfect body to charm husks can create some funny images. Someone forgot to tell to her, that husks don´t have brains, so sex appeal is not good weapon against those things. And shields don´t work, becouse they just run to you and try to beat you to death.

But you should not think these things anyway.
