ME1:
Tonn Actus, a.k.a. Wrex: Family Armor, a.k.a full room of bastards spamming Immunity and running at you.
Liara recruitment early and battlemaster in the end. On Insanity. I had to restart entire level and switch Garrus for Kaidan (so it became Vanguard!Shepard / Kaidan / Wrex).
ME2:
Horizon, first run, Veteran, Vanguard, Praetorian. The conversation right after it adds insult to injury. It became far easier on following playthrough with Infiltrator / Sentinel / Soldier and when I figured out that particle beam has cooldown.
Five waves of Arrival, Insanity, Adept, Reave as bonus power. Singularity eats through enemy shields so slow, you can't Reave for HP them and don't have shield boosting powers either. Definitely Vanguard and Infiltrator-friendly level, though.

Collector's ship on insanity. Still haven't come threw. Little cover, and flanking enemies everywhere while Scions are flying around your platform...

Not having the ideal characters for this particular missions didn't help either (I had Zaeed and Grunt).
Seemed like poor game design to me but I beat it anyway and shrugged it off.

It wasn't really difficult to complete it on Insanity, but there where some part that were a bit more challenging. Overall, I hope Insanity will be harder to complete in ME3.

Collector ship ambush and Horizon Praetorian weren't so bad with Sentinel/Miranda/Mordin, especially with Reave as a bonus power.

OK serious now, I would say Benezia from ME1 because of that stupid glitch that if she hit you that you wouldn't be able to get back up. For ME2, the praetorians on the collector missions were the bane of my existence.

The ones I've always hated were the turian assassins outside Fist's and the krogan on Therum when playing on insanity with a low-level Shepard.

To be fair, I don't think those battles were meant to be fought with a low-level Shepard on Insanity. Which is why you should always play Insanity through NG+, otherwise you might break your game disk in half. They were already pretty challenging on lower difficulties.didymos1120 wrote...

EDIT: As for the Benezia glitch, I've played the game close to 12 times and never encountered it. Guess I got lucky.

Or Arrival, that gave me a run for my money once. I then had an rage/adrenaline rush with my fingers and I beat it the next try.
Everything else was just..simple, as long as you didn't act like a jackass.

Personally, all my most difficult ME1 fights were a few of those sidequest mercs-in-a-warehouse fights where you get rushed by a dozen guys all spamming immunity.
In ME2, it was that last area on Horizon with all the husks and two Scions.
