Definitely the Collector Ship on Insanity, as everyone else has already mentioned. I breezed through the game up until that part, then got stuck on it for like 4 hours, then breezed through the rest of the game without too much trouble.

For me, I always tended to go get Liara first in ME1 and thus, my levelling was a bit low. That SOB Krogan and his henchmen on Therum (I think) always WORE ME OUT!

Definitely the Collector Cruiser on Insanity. I got stuck on that for a while with my Soldier playthrough.

Then I got the Firepower DLC Pack.

Two things from ME1 and two from ME2:

ME1:

Tonn Actus, a.k.a. Wrex: Family Armor, a.k.a full room of bastards spamming Immunity and running at you.

Liara recruitment early and battlemaster in the end. On Insanity. I had to restart entire level and switch Garrus for Kaidan (so it became Vanguard!Shepard / Kaidan / Wrex).

ME2:

Horizon, first run, Veteran, Vanguard, Praetorian. The conversation right after it adds insult to injury. It became far easier on following playthrough with Infiltrator / Sentinel / Soldier and when I figured out that particle beam has cooldown.

Five waves of Arrival, Insanity, Adept, Reave as bonus power. Singularity eats through enemy shields so slow, you can't Reave for HP them and don't have shield boosting powers either. Definitely Vanguard and Infiltrator-friendly level, though.

While not the toughest battle by any stretch, dealing with immunity users from ME 1 on insanity was annoying. Literally, I was dead on nailing the mercs in the head with the mako cannon and seeing them flying off, very much still alive, over the tops and down the sides of mountains. It was just annoying because on casual difficulty, just bumping into enemies with the mako was enough to take them out, and instead of killing them, their weird physics engine has them flying like superman after being struck and letting them land a third of the map away. I didn't even realize that they weren't being killed by the impacts until I notice I wasn't recieving experience and decided to see where they were landing. I literally ran over and started jumping the mako up and down on a enemy so see how long it would take to kill a enemy using that method, 3 minutes of a tank jumping up and down on a guy's head before he finally died, and should it occur to you that shooting him might have been quicker, granted, but as a example it still holds. It should never be easier to kill a enemy on foot than it would be to kill them by either shooting them with a tank or literally dropping one on their head 50 times, so that's the only real time the the game wasn't just tough, but irritating enough that I almost quit the series right there.

Collector's ship on insanity. Still haven't come threw. Little cover, and flanking enemies everywhere while Scions are flying around your platform...

This. I'm suprised I was able to beat the game considering how hard that level is on Insanity. So many times I wanted to throw my controller.

This is very true, thank god I'm done with insanity.

Toughest battle?! Hmm, I guess that would have been fighting the feeling that made me want to throw up over Origin.

I'm currently playing on Insanity. Had a few hiccups here and there, but nothing that took more than a couple of attempts...but now I'm on the Collector's ship and I can't get past it. It's driving me crazy.

Either:

ME1: Toughest battle nominees: Noveria, Matriarch Benezia, or: Citadel Tower, Husk Saren.

ME2: TBN: LOTSB: The Asari Spectre whos name I have forgot, or: Horizon, Praetorian.

Mass Effect 2's Collector Ambush on Insanity.
Not having the ideal characters for this particular missions didn't help either (I had Zaeed and Grunt).

Seemed like poor game design to me but I beat it anyway and shrugged it off.

It wasn't really difficult to complete it on Insanity, but there where some part that were a bit more challenging. Overall, I hope Insanity will be harder to complete in ME3.

Mako and the mountains were a tough nut to crack sometimes, other then that ME2 Insanity was quite easy, didnt like much ME1 combat, its was all like bang, bang, bang with assault rifle.Seriously tho Firepower DLC pack makes it too easy.

I suck at combat, so I've only played ME2 all the way through on Insanity once. For some reason the Warden Kuril boss fight gave me the most trouble. I think I played it 30 times. For the first ten or so I didn't realize I had to take out the far shield generator to stop the commanders from spawning, so I got beaten down in cover waiting for them to stop spawning -- which they never did. I became less incompetent as the game progressed.

Collector ship ambush and Horizon Praetorian weren't so bad with Sentinel/Miranda/Mordin, especially with Reave as a bonus power.

The hardest battle so far for me is waiting for ME3 to come out!

OK serious now, I would say Benezia from ME1 because of that stupid glitch that if she hit you that you wouldn't be able to get back up. For ME2, the praetorians on the collector missions were the bane of my existence.

My toughest fight was definitely my first encounter with Benezia way back when. I was not ready for her and it took me at least an hour and a half, maybe longer, to finally beat her. When I finally came out the other end I was a master.

I've never understood the reputation the Benezia fight got. Unless you had the misfortune of having a biotic attack glitch you through the floor right off the bat, it was pretty simple: book it for the back of the room and camp there until it's just you and Benezia.

The ones I've always hated were the turian assassins outside Fist's and the krogan on Therum when playing on insanity with a low-level Shepard.

I get the feeling the Benezia glitch has simply been experienced by some more than others. Myself, I've hit it a whopping twice, so I don't really care. Others though, they make it sound like a pain to get past or something.

To be fair, I don't think those battles were meant to be fought with a low-level Shepard on Insanity. Which is why you should always play Insanity through NG+, otherwise you might break your game disk in half. They were already pretty challenging on lower difficulties.

EDIT: As for the Benezia glitch, I've played the game close to 12 times and never encountered it. Guess I got lucky.

People keep talking about the krogan battlemaster fight, but it's really not that bad so long as you bring Kaidan (or if you're a sentinel yourself). Neural Shock does wonders against that guy.

Kaidan's pull also works from my experience; you can fly him to the other side of the hall and spam assault rifle fire all along. It usually doesn't even give him a chance to get back up.

Insanity wise: Probably Horizon towards the end. The waves of husks and all that jazz.
Or Arrival, that gave me a run for my money once. I then had an rage/adrenaline rush with my fingers and I beat it the next try.

Everything else was just..simple, as long as you didn't act like a jackass.

Exactly; I don't see why people make such a big deal out of that fight.

Personally, all my most difficult ME1 fights were a few of those sidequest mercs-in-a-warehouse fights where you get rushed by a dozen guys all spamming immunity.

In ME2, it was that last area on Horizon with all the husks and two Scions.
