Krogan battlemaster in Therum on Insanity. It was like he had infinite health. Only frustrating fight in both games.

In Mass Effect, tackling the UNC missions involving groups of biotics was a pain in higher difficulties. The "Sovereign Saren" battle at the end was also pretty challenging at higher difficulty settings. And I sweated some of the Pinnacle Station runs.
In Mass Effect 2, getting to that last hatch to close on Garrus's recruitment mission was a ****, even on Casual. The effort to take out that Geth Colossus on Haelstrom (on Insanity) was crazy. And I learned to hate those damned Collector Praetorians with a passion.

Certain ordinary-ish battles using my Vanguard, usually involving me wanting to charge a dude and the game not letting me, while selecting charge moves me out of cover and into enemy AR fire.

Collector ship seems to come up a lot, so how about we give tips on beating it?
Difficulty: Insanity
class: Sentinel with Reave bonus power, points mostly in Reave, Tech Armor and at least one in Throw
Squaddies: Miranda (Leader/Warp) and Mordin (Scientist/Incinerate) both with Machine pistols
Collector Ambush:
Shep should stay in cover in the middle of the platform, scions can't touch you there.
Concentrate on the peons first, otherwise you will have to kill Harby over and over again.
Warp/shoot the barriers of the collector drones/guardians, then Throw them off the platform. (Instakill with short cooldown = win.)
Have Mordin shoot barriers and repeatedly Incinerate one of the scions. Don't waste Incinerate on peons.
Once collectors are dead, kill Harby (Reave/Warp/Incinerate makes a nice dent).
Attack the scion Mordin was chipping away at before the next wave appears.
Rinse, repeat.
The big thing I learned was not to take out Harby too soon because he'll just respawn in an almost-dead collector, thus wasting all the damage that you dealt to both Harby and the peon. Kill him only once -- when he doesn't have another body to control.
How do other classes handle this fight? Samara could handle the Reave/Throw duty if you're not a biotic.

Collector ship ambush, Horizon, Obj Rho - one thing else - those damn 20 crates - never saved more than a handful

ME1 - Krogan Battlemaster during Liara's rescue. Whenever I thought I was close to having him down a Geth would take me out.
ME2 - Platform battle on the Collector ship. It was enough to make me want to quit my insanity playthrough. I'll have to give a honerable mention to the final Horizon fight though - it was no fun on insanity either.

For anyone having a hard time with Praetorian fights, if you have powers that can regenerate your shields from NG+, pick those. Best thing to do is just keep hiding behind things and staying very far away from it, only popping out to shoot when you have an opening and full shields. It will largely ignore your squadmates, who will help you take it down. Just exploit the terrain and the fact it's large, fat, and slow, and you should be alright!

The praetorian on the collector ship is quite challenging, especially if you get that bug where your squadmates die right before the fight;
My struggles with the menu interface on PC were also spectacular.

Also those three ymir mechs in the "save the crates" side mission whooped my ass quite a bit (not using stasis of course). They still do if I'm low level and don't bring the right squadmates.
