Amnesia the dark decent, probably the most scary game I've ever played.
Dead space isn't a game I consider scary, but I do love the series so far (although I hope they're more subtle in DS3 than how in your face DS2 was) I love the idea of mixing Alien with the Thing, my two favourite horror movies. Dead space suffered from cheap monster in the closet scares, but what Dead space did right was atmosphere, it was one of the most atmposheric games I've ever played! I love the sounds you hear of water dripping from that broken pipe, the lights flashing against two suits that look like two people have hung themselves, and you hear a little screech in the distance. Dead Space would be the best horror game ever made if it had more subtle scares combined with it's atmosphere. (Keep in mind I always play games with headphones)

That moment in the game scared me half to death. I never played it again after that happened. I will never forget that moment of terror gripping my soul. I sometimes consider going back to finish it, but then I remember...and suddenly I don't want to touch it again.
Phantasmagoria - I played this one when I was around 10-12yrs old and it messed me up big time. I had nightmares of some of those scenes after I had played around with that one. Great game but I was way too young to handle it at that point in time. Curiosity is what did me in there.
Haunting Ground also did a good job of getting my heart racing. By the end of the game I was more annoyed and frustrated than scared, but with the first two bad guys I was pretty scared. The second one especially...I think what bothered me the most with her is that she didn't just go after me, but she would attack Hewie too. The fact that my character would tire out and trip on things when running away really didn't help me with the feeling of fear I got from that game.
As far as "disturbing" goes however, that prize goes to Rule of Rose. If you've played that game before, you know what I'm talking about. Another word of caution to those out there: Animal lovers -- do not play that game.
Edit: Forgot to mention Kuon. Good scary game with some interesting gameplay elements. It had a great method of building up fear through what you could hear but not see. The fact that you also only had a limited number of saves made the battles much more frightening as well.

Every once in a while something unexpectedly gets through my defences, though. I think the games that actually succeeded in freaking me out are the underground sections of Stalker and oddly enough boring old Doom 3, at least in its quieter moments. Dunno why, considering the widely acknowledged scary stuff has zero effect on me, but those two scare the crap out of me.

Haunting Ground was pretty creepy, I suppose. Kinda offset by the woman running around in nothing but even that served to heighten the vulnerability. Especially when the lumbering brute comes charging out of nowhere.
Oh, can't forget to mention Clocktower.
