I haven't played in months and the disk as been sitting safely in it's case this whole time, I go to put it in today and the xbox won't read it. I look at the back of the disk and there is a huge scratch all the way around it! UGH!
If I get another regular edition, will all my sig items like Sebastion and the dog remain unlocked on my console?

The items are attached to the console they were downloaded to so you'll be fine. The Signature Edition disc is identical to the regular disc as everything extra is downloaded, just as you would do if you bought the regular edition and bought the DLC from the Live marketplace.
