After what Shepard has done for humanity and the galaxy, I don't think there is a rank in the Alliance Military that would be fitting for him. Nobody knows more than him about the Reaper threat, nobody has done more than him to stop the Reapers, so I don't think anyone of his crew is going to question his leadership.

Even if Shep is a spectre he still part of the Alliance or will be part of it. . .but as far as the Alliance goes its combination of navy & marines . . . .navy being the fleet ships and marines being the ground force. . .check the Mass Effect Wikia or the codex. . . Kadian does out rank Shep in ME3, in ME1 was under Shep in rank. . . If you go to www.militaryfactory.com it gives you all the ranks for ask the militaries

Unlikely as a Spectre he/she works for the Council, not the Alliance. That would be the same as if the Turians or Asari or Salarians promoted Shep. Shep may be an alliance soldier in his/her heart, but not as a matter of fact.

The official term is "Time in Grade".  Assuming Shepard "holds" the same rank he/she had in Mass Effect in Mass Effect 2 (Lt. Commander), Kadan being promoted to Major/ Lt. Commander would still mean Shepard outranks Kadan due to time in grade even without the three years Shepard was MIA/ KIA.  However certain freely available footage implies Shepard gets promoted to full commander - equivalent of  Leutennant Colonel.

There probably won't be anything happening. I imagine that there might be a dialogue joke about out ranking bit but it probably won't go anywhere beyond that.

If its a Naval comparison, Commander would out rank Major. At least that's the case with most of the militarys I've been exposed to.

They changed it mainly due to one of the lead female characters in novel series who has the last name Sanders to avoid confusion who we may finally meet in ME3 at least I hope we do.

Kaidan is dead, not just dead but he's been reduced to radioactive particles scattered all over the planet Virmire, so he won't be giving any orders.

It would if Major and Lt. Cmdr. were actually equivalent ranks.  They are not.  Major outranks Lt. Cmdr. (and Staff Commander) in the Systems Alliance's ranking system.  Read the Codex. Or this thread, wherein the Codex entry has been posted.

Where are you getting that from?  I don't think they've ever said why they changed it.

Wow, it's amazing people are still getting excited about this whole "outranking" issue. There are several fine examples of a working relationship between Shepard and military officers who outrank him (Anderson & co.) and I don't see why this one would be any different. I'm pretty sure military ranks won't mean much anyway.
