Suicide Mission bug (spoilers)

I'm hoping there's a way to remedy this that doesn't involve a new game. I'm playing through both Mass Effect games to make sure I have the best story for ME3. I've beaten the game before with all survivors so I know what to do to ensure their survival.

I've never experienced this bug before, but now whoever I send into the vent dies. I have loyalty for all members including the DLC ones. I've tried with Tali, Legion, and Kasumi and all are killed in the cutscene. Obviously I want all members to survive, so is there any way I can keep them from dying?

P.S. I don't know if anyone else dies when they shouldn't as I haven't gone any farther.

Yeah, I just played through on Insanity and while I still completed the game anyone who was in the vent died. My first playthrough everyone survived with minimal effort. There must be a problem but I'm not sure what it is.

Not aware of any Suicide Mission bug myself. I would hazard a guess that it is more likely because you chose a poor Fire team leader that leads to the tech expert's death. I assume you've already checked this in which case you can confirm whether or not you made the wrong choices.

Never heard of any bug but try choosing either Garrus or Miranda to lead the other team, they are the most capable leaders. I usually go with Garrus as leader and send Legion into the shaft (Tali works to) and everyone survives for me.

Except for the first time I ran with the same people and none died. Explain that.

That's exactly who I used in the shaft, Legion, and guess what? He died.
Full loyalty on everyone, so that wasn't a problem.

So who did you have leading the other team? That is important. If you have a "incompetent" leader in the other team the one in the shaft will die. Don't know if it matters at this particular point but if you're playing on Insanity you also have to be fast for everyone to survive.

Edit: Are you sure you didn't unloyal anyone? How did you handle the fight between Legion and Tali? If you didn't have enough renegade/paragon points to pick one of those options (i.e neutral ground), you have to choose a side and if you picked Talis side then Legion is not loyal anymore and vice versa. If this is the case then you have to talk to Legion again and say you lied or something to get him loyal again, same with Miranda and Jack.
