Shep and co., after 80 hours of playgame, are unable to find a "cure" to the reaper threat and thus the Reapers win. It ends in a "whisper", since it will pass many years, perhaps centuries, before all traces of civilization gets erradicated. Shepard and some friends will see this doom unfold quietly and slowly, from a relatively secure vantage point. Until some reaper finds them and destroys them. When that happens though, Shep is already defeated in his / her mind, and accepts his / her death.

The fate of the galaxy actually depends on whether or not Garrus survives the suicide mission in Mass Effect 2, so he'll have time to finish those calibrations. If he finishes the calibrations he'll use his MEGA DEATH LAZ0R to destroy the reapers. If he dies in the suicide mission, we're stuck with shepard's plan "We fight or we die" and sadly that would end in death.

Making victory depend upon the survival of some ME2 character would be very very funny indeed.

I'd vote for Kelly Chambers, for it is so easy to let her die in the first runs of the game.
