Upon entering the orphanage and getting all the way into the back to fight the demon, when the demon dies
both doors to the room I am in are still locked. I get the "we won" conversation, but the doors never unlock. There is no key, I have loaded my game and done this event 5 times now, only to still be locked in that room.
Anyone else had this problem and gotten out of it?
What a tragedy.

i am also having this problem.
At getting to the first demon i was locked in that room, couldnt talk to the npc, no way out. Then i loaded from a previous save and went through it again, but this time the NPC didnt follow, when i went back to get him he wasnt there, i went all the way through the orphanage killing some minor demons then exiting, my quest is not yet complete and now the templar NPC is gone.

I had this problem as well, so i left my controller alone for a while and sure enough it went to a cut scene, this was due to the Memory leak within the game which i hope Bioware fixes.

I had a similar issue with the Battle of Redcliffe.
I had been playing for several hours, after 3 or 4 waves of zombies right after starting the battle, they just stopped coming. I ran all over the whole town, nothing. After waiting about 10 minutes, I just quit and reloaded my previous save, and then the battle went as intended.
The memory leak is really quite ridiculous. All of these issues had to be known from testing .. its just absurd EA/Bioware released this game in its current state.
To add insult to injury, here they are releasing DLC before patching the game you already bought =/
Really, this game has ruined my trust in Bioware. ME was a fantastic game when it came out, I never ran into any of these sorts of issues. Im now very scared for the state ME2 will be in when released.

clearing your 360 system cache may help improve your gaming
however it also deletes any updates for any games you have (your dlc will be safe)
just play a game again to recover the updates

I know that as long as you wait and don't press a to talk to the Templar you won't have to worry about the doors staying locked because the victory cutscene will eventually pop up, I usually run around the room in hopes of speeding up the trigger. I also noticed that sometimes you'll end up with a debuff that won't let you exit combat mode until it wears off. Perhaps this has some effect on how long it takes for the cutscenes to trigger as well.
