Looking over the balcony after the first major battle, there is a dialog about the dragon's teeth visible at the end of the platform - I had never found it before my first insanity run - but redoing the mission (decided to change up squad) it is not available - any clues?

In order to get the dialog, I believe you have to have Garrus, Tali and/or Jacob in your party, since the dialog triggers due to reminiscing about Dragon's teeth from ME1(Jacob is included because he was on Eden Prime or something).

i kinda suspected it might be that (interesting that Jacob gets included with the ME1 squaddies, but not Miranda) - I had taken tali first - and was not happy with the choice - so respec'd w/ Mordin/Samara - a better crew for a sentinel insanity run and CC, i hope...hate husks.
