First I'd like to say that I didn't play Dragon Age Origins, but I have read the plot summary in Wikipedia.
In the end, Cassandra says that the champion is "gone, like the Warden". According to what I've read about the endings there are 3 endings: You die, you let someone else die for you, or you have a child with The Morrigan. Disapperance is not mentioned. Is the Warden she talks about the character you play in DAO? Regardless, can anyone explain the Warden part to me?

Nope. None of the previous games allude to the Warden protagonist disappearing after either DAO, Awakening or any of the DLC (except maybe Witch Hunt, but that's a different story).
Regardless, its supposedly meant to be a "cliff hanger" where we are left wondering what the heck is going on. Which it seems to be successful at.

Actually I think in the Queen Cousland epilogue (haven't played up to any others) the Warden disappeared.
Epilogue cards are notoriously inaccurate as to what's going on anyway.

Good question. It might be explained in the anime movie coming out next month.
Then again, Bioware is becoming pretty famous for bringing up plot points that don't show any sign of getting resolved, so it might not.

Or maybe Varric is really Uriel the Archdemon, dreaming it is a dwarf, right before it wakes up to begin the assault on Denerim at the end of DAO.
That would be a pretty good way to erase a lot of the incontinuity people complain about in the series.
