Story-wise, there are absolutely no repercussions at all. No-one cares or even notices. That, for me, is a good enough reason not to use it.
But if that doesn't bother you then...
Quite a few people said it was vey powerful, though it did get nerfed in the lastest patch. Personally I've never found it terribly useful. Not being able to heal is a pain, and sacrifice seems to require you to always bring a reasonably sturdy companion with you (ie, Aveline).
Personally I'd give it a miss. It completely jars with the story and doesn't seem all that great anyway. There are plenty of great mage builds that circumvent it anyway.

bm/fm are the optimal mage specs. if you're going for efficiency, those are the two to take.
casting from your health pool alone is worth the price of admission, the awesome spells (blood slave, hemorrhage, sacrifice, etc) are just icing on the ****** cake.

pros.
- blood magic allows sustains to be held without impacting the casting pool
- blood mages can (and should) skip willpower, investing into constitution instead. this means that compared to the typical high willpower alternatives, a blood mage will be more durable and can afford to pump more magic.
- the spells from the blood mage tree are quite powerful.
cons
- there are only two methods for healing while using blood magic (that is, using your companions or enemies as blood banks). certain situations will require dropping out of blood magic so a potion or heal can be used; and as several sustains are probably been used and the mana pool isn't very large to start off with, hawke is unable to cast until going back into blood magic
- it is necessary to pump constitution or risk death via spell-casting. this means that glassier pure magic builds are not as viable (e.g. using death syphon for mana)
- the blood mage tree requires considerable investment to gain full effectiveness; this means that less points are available for other things.

A personal pet peeve of mine, that made me hate the Blood Mage spec for ever.
As a blood mage, it only makes sense that you load up on sustains to make use of that mana, you otherwise don't need. The thing is, once your mana pool is reserved for sustains at 90% or more, your Hawke starts to whine like a little b**ch about every 10 seconds or so. It got so tiresome to listen to "I NEED MANA!" and "I CAN'T KEEP THIS UP!", that I was forced to either endure it for the rest of the game, or drop some sustains, so that my mana pool was reserved at 80%. That meant I could only use Rock Armor or Elemental Weapons (two sustains I didn't need at all) on top of BM. And 20% of my mana pool was just sitting there being useless, just so my Mage would keep quiet.
/hate
As for the spells, I never found them essential/useful enough to make me take the spec. Usually, I found it was a better alternative to take Creation tree (haste + heroic aura) instead of BM.
And don't get me wrong, I'm not saying BM doesn't have it's advantages, it's just that there are way more con's than pro's as far as I'm concerned (especially post-patch).

I rather like the blood magic specialization. I spent the first ten levels filling the spirit tree all the way, got rock armor and arcane shield mixed in there, and then put the rest in blood magic. What's cool about that is that you can start a fight with BM active, cast hemmorrage, grave robber and blood slave, maybe spirit bolt and walking bomb, then deactivate BM and turn on Death Syphon to finish up the fight and get healed if necessary. I'm basically playing a tank mage and hemmorrage straight up kills guys with little health bars and severely wounds normal and higher enemies, add a walking bomb in there and you can kill several enemies before the fight really even starts. This is on normal difficulty, so I can't speak for nightmare. I'm gonna try some fights in nightmare though to test my tank mage's mettle.
It is annoying to listen to Hawke complain about needing mana throughout the whole fight though, as Relix28 said!

Forbidden magic is the best magic!
Having more health than Avaline and being able to cast until the cows come home, are worth the price of listening to Hawke complain of low mana every fight.

Alternately get the no cost sustains mod, then have at your blood mage w/o the complaints. I love playing a blood/force mage. Overpowered at Normal mode, in casual enemies die before you even cast LOL More than once in casual I have walked around a corner to see red dots vanishing form the map at my very approach. Normal he's still OP, hard is easy and nightmare is a good play for him but, be wary of the AOE spells, they kill companions as fast as enemies.
