Normal difficulty for at least the first hour. After I get used to the gameplay alterations I'll adjust depending on how much of a challenge the game is, though I'm expecting to raise it to insanity unless Bioware have learned how to make good opposing enemies.

In ME2 I only played at hardcore and insanity (second playthrough) but I am considering insanity for my first playthrough this time.
I found that in ME 2 Insanity was frustrating mostly because of the weak design choices but now with a better melee, better AI for your squad and ammo replenished after every mission I believe that I won't have problems.
P.S.
Do we know if Insanity needs to be unlocked?

I usually go with Veteran for a first playthrough or if I haven't played the game for a while, then Hardcore once I'm a bit more used to it.

Go for Story mode on Insanity. It'll still be hard but an EASY hard.

Thinking now to stick with Hardcore. Apparently it will be harder than hardcore in ME2, so wont be a walk in the park. Also leaving insanity for a future playthrough will add to the replay value.

Well it's got to be either story mode narrative/casual, or RPG mode insanity.
There's value in playing first for story, to finish and be able to discuss it freely without fear of reading/seeing spoilers.
But it depends how hard insanity is, and how much time it would take to finish it on insanity.

Does anyone even know if there are difficulty settings outside the confirmed " Action, story, RPG" modes? I'm a bit worried that RPG will be too easy if that's the case, I've always played my mass effect games on insanity.

I'll probably play on Normal or the one just above normal. I don't really like unloading shot after shot into an enemy only for them to not die. It makes the weapons and powers feel weak and crap. Foot soldiers can't be givin me trouble when there are Reapers to deal with.

I'll start on Veteran, and then depending on how hard that is, I'll either go to Normal or Hardcore, or stay on Veteran.
Re-playing ME2 on anything below Veteran to me is a yawn-fest.

Doing Veteran. I am a vet of the series and went from Normal to insane in ME2 and had a little issue but not much.

Well, this is what the new difficulties will probably be:
Story Mode: Casual
Casual: Normal
Normal: Veteran
Veteran: Hardcore
Hardcore: Insanity
Insanity: LASO
You can make your mind up from that. I'll try my hand at the new normal first.

I really enjoyed playing the combat in ME1 - but still never really spent much time on higher difficulties.
In ME2 I didn't really enjoy the combat so kept it on easy to whizz through it.
The combat in ME3 looks really fun - so will start at insanity or whatever the highest is in the demo - and dial it down until I find a happy a balance and play that for the main game. I expect though to end up at the easiest as I become more interested in the story.

Hardcore from start to finish. Will go for 1 death every 3 missions minimum. Any more and I restart.
