If you choose to save the keep instead of the city to those you leave behind assume their normal death states if you didn't do their side quests/get enough approval?

If you save the Keep then you and your companions return to it and all the Wardens survive.

Just to be sure: if i choose to save hte keep everyone that I don't take withme to the final fight will live ?

Correct. You will take your team back to the keep and all the wardens will fight together and all will live.
