Mass Effect
Custom male: Kedrac Shepard, Earthborn war hero. I have a soft spot for these types of 'accidental heroes'; fellows who aren't really planning to be heroes, but wind up in circumstances that make them such and enjoy the fame that it gets them.
Noveria: Spared the Rachni. A race of giant telepathics would make great allies if we need them!
Feros: Saved the Colonists, charmed Jeong. A soldier's duty is to protect the innocent, and slotting an Exogeni pencil pusher doesn't gain anyone anything. Let 'em all walk, and chalk it up to another moment of out-of-control science.
Virmire: Charmed Wrex and rescue Kaidan. Wrex was a trusted mate, and Kaidan was with the salarian team. Saving him meant saving several other soldiers as well. Best solution.
Romance in ME1, Ashley. Tragically cut short by Virmire, but I have another soft spot for heroes that can slot their love interests in the name of the greater good. It's relatively easy to give your own life for a cause. Giving someone else's life for that same cause, particularily a loved one, is much harder.
Morality: 100% Paragon, not sure how much Renegade.
Citadel: Saved the Council, Anderson as councilor. Protecting the Council is probably the best means of ensuring galactic peace, which is a Spectre's primary mandate, and Kedrac promised Anderson that he wouldn't forget about him helping the Normandy get off the Citadel. This is that promise made good on.
Mass Effect 2
Freedom's Progress: Veetor goes with Tali. It was too early on to trust Cerberus not to pull out Veetor's fingernails if he didn't spill the seeds, and the poor kid was in pieces anyway.
Samara loyalty mission: Morinth dies. Not matter how I try to spin it, I just cannot justify killing Samara, who is already sworn to your service, for the sake of a sociopath who may or may not be as good as her mother. Even in my most hardcore renegade playthroughs, I just can't justify that decision.
Tali's trial: Charm the Admiralty Board, speak against the war with the Geth, evidence withheld. Tali didn't want the evidence shown, and this was her mission, so Kedrac honoured her wishes. He still had the ability to prevent her exile, so he took it. Everyone wins!
Mordin's loyalty mission: Maelon lives, data on genophage cure saved. The genophage was a bad idea, and my Shepard still blamed himself for destroying the Virmire cure. Saving Maelon's genophage cure data seemed like atonement, and what does killing Maelon gain anyone?
Jacob's father: Killed on Aeia by the wild ones. Life in prison doesn't help Ronald's victims, and this way at least some of his victims can take out their anger on the bloody tosser.
Thane's loyalty mission: Joram Talid lives, Kolyat doesn't go to jail. However much the aliens tell themselves that humans are out to get them, we can prove we're better than that. So the politician lives, and keeping Kolyat out of jail is what Thane wanted, more or less. So my Shepard made it happen.
Zaeed's loyalty mission: Vido Santiago dies. It was Zaeed's mission, and thus it was his decision. Shepard respected Zaeed enough to honour his decision, and Vido swallowed a bullet. Sort of.
Garrus's loyalty mission: Harkin gets a bullet in the leg, Sidonis dies. Putting a round in Harkin's leg is the least he deserves, and Garrus wanted Sidonis dead. My Shepard respected his old friend and trusted advisor enough to honour his descision (notice a trend yet?).
Miranda's loyalty mission: Niket gets shot by Miranda, Miranda talks to Oriana. Miranda wanted Niket dead, and it was her descision to make. So my Shepard didn't stop her. However, he understood what it feels like to not have a family (growing up on Earth did that to him) so he convinced her to get in touch with Oriana.
Jack's loyalty mission: Aresh lives. Jack was decidely on the fence about this, and that gave my Shepard the 'right' to influence her decision. She really did need to let all of the past go, and sparing Aresh seemed like the best way to make that happen.
Derelict Reaper: Legion is kept aboard the Normandy and reactivated. My Shepard was genuinely curious about this friendly geth, and then jumped at the chance to secure a new ally against the Reapers.
Legion and the Heretics: the Heretics are re-written. Since Legion couldn't choose, Shepard did it. More people against the Reapers are always good, and thus the re-writing decision.
Romance: Jack. Both her and my Shepard came from gang backgrounds with no-one to look out for them, and she was essentially an image of what Shepard could have become if he hadn't joined the Alliance. So he sort of struck a cord with her, and took it upon himself to restore her faith in humanity. Along the way... yeah.
Morality: 100% Paragon, Not sure how much Renegade. Much more than ME 1, I'll tell you that!
Suicide mission: Garrus leads the first fire team, Tali runs the vents, Legion escorts the crew back (all survived), Miranda leads the second fire team, Samara holds up the biotic field, Garrus and Tali come along for the final boss brawl. Everyone lives. My Shepard couldn't bring himself to take anyone except for the old gang along for the end.
Final decision: Collector base is destroyed. Anderson's words rung in my Shepard's head as he stood there pondering: "You can't trust Cerberus".

Curious how many of those 100% suicide missions were do-overs. :-P
