I had that issue, I only got one go. My little brother got it on the games very first try. He hasn't even played the previous two games and he's used it already. Wouldn't be so bad if I still lived at home.

Apparently at first you got a code just by pressing the buttons. There was no "correct order", but then they changed it. WTF UGHHHHHHH

I'm 0/12 attempts. But I'm not letting it get me down. Haha, I've got another three attempts in about four minutes. I haven't even won a Normandy Avatar Prop. Though I did win one through the Mission Command App for Mass Effect 3 and get one through the N7 Edition. So who knows? Maybe I just jinxed it and will end up with a third ship code, and no early demo access. =P
