If you upgraded a power and click "Back" or press ESC before the icon animation finished, you will be brought back to the ESC menu instead of the squad menu. Tried it on shepard and squadmates.

Good spot, I've noticed this issue as well but couldn't figure out quite why it was happening. It's a bit confusing.
