I have seen some trailers that hinted the action is possible, but I can't seem to find the right key combo. Since hand-to-hand fighting tend to happen more often in ME3 due to the aggressive AI, I always find myself trying to grab an enemy that is rushing around a corner or across a railing, but hitting the melee key while sticked to cover yield no results what-so-ever. Manually unstuck myself and then hitting the melee key is just too damn slow and often get me killed.
(If the melee key can act as a secondary "unstuck-to-cover" key that would improve the combat control a lot IMO.)
Is there a way to perform melee action while sticked to cover?

If the enemy is on the opposite side of the cover that you are at; a cover you can jump over, you will see a "fist" icon. Press melee and you will hop over and swoop down on him.

What about in other situations where the enemy is not on the opposite side of a climbable cover (around a corner, snuck up from behind, etc...)? Is there a way to perform a quick melee/tackle/stun without manually breaking cover first?

I don't know about the icons because I turn those off. So tired of that kind of
hand holding. I can tell you this though.
You can do the Heavy Omni Tool Melee from cover as prieviously mentioned.
You can go to the corner of cover and do Light and Heavy Non-Omni tool attacks
from corner cover if an enemy is there. You can test it even if there are no enemies
so you can see what it looks like and get the timing.

No, it becomes visible as soon as an enemy becomes close enough and is not fully aware that you are in that piece of cover...which is rather rare on higher difficulties unless you play as the infiltrator.
