I also had the exact issue.It was resolved via the mic boost sky high setting.............
Windows 7 with an Realtek HD audio card (can find the chipset if needed) , with a common headset of logitech

People with newer Soundblaster cards can't use the 20-40db "mic boost"; the option doesn't exist. Yet my mic works fine on Ventrilo, Skype, and other games without it. (I usually end up setting myself to 30% volume because I'm so loud!) But in ME3? Nobody can hear me at 100% and shouting unless I eat the mic and shout on it. Even then it stops detecting between words and cuts out.

The problem is the Voicechat's "detection" system --  that is the route of all evil. Apparently even with Push-to-talk, while holding the button you're not broadcasting, you're actually just queueing a detection to SEE if you're talking (the way "always on" works -- loud noises above the norm are detected and all others are discarded). This is why the Voicechat system doesn't work.

If they *removed* the detection system from Push-To-Talk, I bet we'd see a lot of people using their mics.

I've never been able to get voice chat working in ME3. I really wish BW would fix it, it's infuriating.

For me it worked fine until I reinstalled Windows. Then it stopped working. Reinstalled Windows again after some time, worked again. Reinstalled after some time, broke again. And yeah, I'd really love a chat system. They could simply remove that stupid frag count spam. We already got XP showing in the center of the screen.
