Bioware has played fast and loose with the whole rank and title thing in the game, basically (e.g., see what Durin Duranbah says above). The rate of promotions is also ... unusual, but I guess that could be justified by it being an "unusual" time (e.g., see what WalkertheWannabeCoastie says above). So it's hard to rely on any of this in terms of consistent canonical narrative ...

But Shepard isn't always a Spectre at the beginning of ME3.
Keep in mind: Shep lost his/her Spectre status when s/he died. S/he can get reinstated if Anderson is Councilor (but can refuse it) and Udina won't even bring up that option. So the chances are high Shepard won't be a Spectre at the beginning of ME3.
Shepard holds the rank of a Lieutenant Commander (although no one calls him/her Lt. Cmdr.) So s/he also could be Staff Commander in ME3 and the promotion never got mentioned. Aaand: when Anderson reinstates Shepard, he calls him/her "Commander" - not "Lieutenant Commander". Field promotion anyone?
Even if Shepard is "just" a Lt. Cmdr., s/he outranks Ashley by experience, regardless of Spectre status.
Also: Major = Captain. Kaidan outranks Shepard. However, since "Major" is no navy rank, Shepard can still be Commander of the Normandy since Kaidan is NON-navy personal. In extreme cases, a Captain can give orders to a General as long as the General is on board of a ship.
I just don't get it when Ashley gets transferred from marines to navy. Maybe she got recently, before becoming CO/XO of the Normandy? In that case, I'd say Anderson did the paperwork.
Also at the start of ME3 Shepard was stripped of his/her rank and spectre status (if the player got it back in ME2) due to the events of Arrival, The Alliance dosen't want to start a war with the Baterian's since it would make the galaxy even more weaker when the Reaper's arrive and Spectre's are ment to keep the peace not start wars
We thought that spectre was stripped because the demo apparently had Udina picked or Shep turned it down. As we see now if Anderson was picked and offer accepted, those plays have Anderson say Shep is still a spectre.

Um... I agree but there will be someone who will say that if a Commander in rank is the CO of a ship, they are addressed as "Captain". But then his going from Commander (and Shep is supposed to be a Lt. Cmdr at that) to Admiral might actually beat the record now held by Ashley Williams who took it from Kirk who took it from Johnny Rico.

Also two more things:
1. at the "ceremony" when you become a Spectre the Salarian councelor says:"...those whose actions elevate them above the rank and file..." (so I guess being lt. commander or captian doesn't really matter for Shepard)
2. Rear Admiral Kahoku wears the same uniform as navigator Pressly .oO further proof of inconsistency

I noticed the inconsistency and IMHO there is a mistake in the codex. Kaidan is a Major, but with 2 gold stripes his rank should be considered as equal to Lt. Commander, only more specialized (in clear conflict with the statement of Anderson in ME2 about his promotion to Staff Commander though). Shepard on the other hand is shown to have 3 black stripes in the ME3 dress uniform, making him a Staff Commander and the highest ranking officer on the Normandy.
