Re read the OP, i never stat anywhere that the insanity level is "too" hard. I enjoyed it and didn't get much trouble with it.

I had some of the same problems. I also had a problem with the amount of grenades they throw. There was a few times that I dodged one grenade only to have to dodge another.

same problem on insanity... I just gave up in the end, dumb down the difficulty to get use to the control. i'd gladly die by grenades.. when and if only i fail to spot them. but there are so many times when i've actively avoided them, just to be caught by the sloppy controls, and die anyway.

Re read the op, this is about external problem making your dodge fail.
Every time i mention getting killed by a grenade it is because the dodge fail half way thanks to a tiny collision on a flat wall that forbide you from getting away, and once the icone from a grenade throwed from a dead angle didn't showed up.
All other grenade was dodged just fine... when there is no external problem compromising my move.

To anyone saying: "If its too hard, then turn down the difficulty"
Its not an issue of difficulty, its an issue of getting killed randomly because the game isn't giving you the tools you need to properly avoid the grenades.
Issues I had....
1: The Grenades Themselves are too Hard to See: The indicator that someone has chucked a grenade at me is very easy to see; but where the grenade itself is can be very hard to spot. On one occassion I saw someone chuck a grenade at me; but didn't see where it landed and the on screen indicator was telling me that there was a grenade nearby. So I rolled backwards to try and get away from the grenade and rolled right into the damned thing as it exploded unaware that the dude had thrown the thing behind me.
Suggestion: Make the grenades easier to see and/or make their trajectories more predictable and/or make the grenades beep if I'm within the blast radius. I never had trouble spotting the grenades in Lair of the Shadow Broker and when I got hit by grenades in that DLC I always felt like it was my fault, not the fault of the game's design.
2: The Camera Can Make Evading the Grenades Frustrating: QUICK! Someone threw a grenade at your feet! You roll away....and you roll right into a piece of cover that was sitting right off screen.
Suggestion: Once again make the grenades more like the ones in Lair of the Shadow Broker. Their longer countdown made them easier to avoid; but they still served the purpose of flushing you out of cover.
