Not too hard to take out (with several if not all classes) but a huge threat when you don't pay attention and/or they sneak up on you.
Pretty much this... whenever the "Phantom!" yell goes out, I tend to find it works best if group focuses on hunting her down... course it'd be nice if there were two on the field if the callout was "Phantoms!" that way groups wouldn't die to 2nd one whilst dealing with the 1st, kinda like group I was in earlier today did
To the OP query about single-player, I say... BRING IT. Plenty of instances where Shepard can do 1-shot/hit kills so don't see why we can't have an enemy that doesn't if we stupidly let them.

easy way to beat them is warp, tech drain, and anything else you can do to get there shields down and then use either with a max wide singularity area so that if you do her her sheilds down and if you have adept or mechanist to help burn them down, do not ever fight then in close combat run away and use anything you can to get them off and or knock them away from you

Yes, I believe the age old adage of "Never bring a gun to a swordfight" fits this scenario, sure we have our 'Omni-blade' but it doesn't match up against her Katana's (or whichever swords it is she is wielding)

There are plenty of good ways to take down Phantoms. Easiest way is having an Asari cast stasis on them. Shoot them a few times and then detonate the stasis with a throw. Done.
Vanguards with low enough cooldown on charge can basically stunlock them between than and their other abilities.
Several class/race combos have overload; that takes care of the barrier, and the Salarian infiltrator's energy drain does the same thing.
Automatic weapons fire will keep them dodging rather than approaching you (although on higher difficulties, this isn't ideal either because even at a distance, the phantom's palm blaster does a lot of damage).

So much of this. Or let your teamate unleash vengeance upon them on higher level while you put stasis elsewhere.

Once you gain a few levels they're really quite manageable. At lvl 1 however you should probably keep your distance.
