It's a bug. The only way I fixed it was leaving the game.

Happened to me when I switched from my Sentinel to my Engineer in the lobby. Spent the whole game only using Incinerate xD

Edit: I should add, my quick slots were empty besides my Incinerate.

Apparently its a problem if you switch characters in a lobby. Leaving the lobby and re-entering should fix it.

Yeah. The bug occurs when switching characters in the lobby.
In other words, don't ever switch characters in the lobby. At the least rejoin the lobby.

It happends when you change character in the lobby. Best is to choose your character before you join a Quick Match.

If you do end up getting it best solution is to exit and rejoin game.
