Falling through the world

Is anyone else having this problem? Is it an issue with charge not putting the character in the right position or is it an internet issue and the game is having a hard time registering where my character is at any given time? It's really frustrating to die because your character is stuck and even more frustrating when someone revives you and you die right away afterwards. I feel like I'm letting my team-mates down a lot because of it. If there are any helpful solutions out there, I'm more than open to trying them (so long as they aren't "Stop playing Vanguard!" suggestions).

I just wanted to let you know that we are currently investigating this issue. If you have any additional information on what exactly you're charging (platform height transitions/near railings or any other sort of geometry). If you have a spot where you find this is more common please let us know as well. Thanks!

Edit: It also might be worth mentioning that the NPCs don't seem to register me being out of place. Even when I'm entirely under the map, I can still get shot and killed. Dying also puts my body within reach of my teammates, so it seems I'm only out of place on my end.

However, there are times just walking around the map (usually after a "storm") where the entire world stutters around, this I assume is a result of lag and usually clears up if I stay in one spot for a few seconds.
I definitely notice issues while charging at the same time as death that Focant has experienced but my biggest grievance has been the random repositioning of my character after a successful charge when she hasn't been close to dieing.
Thanks for keeping an eye on the bugs, I'll try to give more detailed feedback as I can. Curious if any non-Vanguards have had this sort of issue too.

- At a doorframe charging to a target visibile through where the door would be, caused stuttering and phasing back and forth for awhile but cleared up after not moving for a few seconds once target was dead
- The L-shaped concrete blocks in the "middle" of the area have caused lots of issues while charging from cover, these often cause me to bug out and my character shifts from being in cover behind them to being some where else, any time my character phases through these objects there is usually some stuttering going on afterwards
- This was an oddity that didn't involve charging: using the cover behind the railings near where the cars are all parked and aiming at a turret on the other side of the stairs triggered a fall-through-the-world effect, wasn't actually in cover but about to go into it and take a few shots at the turret and was stuck for the rest of the match (sorry team-mates!) falling through and sometimes above the map
- Fairly sure this is the result of a bad connection to the host since there was mild stuttering before falling through but here's a description anyway: randomly walking through a room looking for something to shoot and fell through, the difference being that this resulted in an eventual freeze up (first time experiencing that) and I was still revivable by team-mates despite being frozen on my screen. Image here. No Fraps sadly, though this one felt very random and unconnected to the charge issue altogether. Was unable to claim rewards at the end of the successful mission when this occurred (stuck on loading..) so this further reinforces the idea that the connection was bad and was perhaps lost towards the end of the match.
From what I can gather so far, there is zero issues with charging in the open (railings and other more or less open objects are a non-issue) it's only when solid cover is a variable that things can sometimes get hairy.

The glitch occurs when you charge while dead.
The server and client are out of sync slightly(lag?) and you have to have pressed the charge right when you die and the server thinks you are dead but your client still does not know this. I then believe the server and client cannot agree where your characters true position is and the discrepancy only gets worse with time until you are floating way above or below the battlefield on your client but the server shows other players your position normally on ground near them.

I've had the vibrating followed by falling off the world a couple times now, and I also just fell through the world without the vibrating, which was a first.
Can't remember who I was playing the past times, but it's definitely not just Vanguards. This most recent one I was a Salarian Infiltrator.

Then the fun times start. You can start flying up or down, get killed in weird places or get shot from strange angles.
