I was the last player standing and a dirty phantom back stabbed me, I was dead. Then during the pause time before the game ended, I appeared alive below the floor floating gradually downwards - shaking somewhat. Good view of the lower rim though. First glitch I've come across

I've fallen through the floor in single player when charging to a target on the stairs. Quite a surreal experience.

Happened to me to used a medi gel t revive myself and fell through the floor ended up failing the mission 1 wave before the end

It's happened to me many times. The greatest thing is that your team mates get really quick regeneration on their shields as a result of this. 
We finished a silver round quite easily because I was floating in mid air while their shields made them almost invincible. (Almost, because phantoms, mini guns and grenades can still get you - all else you can walk toward like the Terminator).
