The weapon models are black for me on the modification screen, clicking change view fixes it however, and clicking it again so it returns to the default view, the weapons is properly shown. Is this a known bug or is my gfx card acting/dying?

This occasionally happens to me with the avenger. Nothing else, just the avenger. Happened with Human Sentinel.

Having the exact same issue. I think this is also linked to the veil of darkness which covers some faces during the cinematics.

I've had it happen as well. Usually for the heavy pistols (Predator, Phalanx, Carnifex). Haven't noticed it with any other weapons. My class is human male Adept. Also occured with asari Adept.

It doesn't just pop up for a certain weapon or a certain class.  I've had the glitch occur with all but the Engineer class as that's the only class I've yet to make a character with.  It's happened with all of the weapons I've locked, whether they're modded or not.  There's no one thing I can do to replicate the glitch, it just happens.

Yeah it just happens seems kinda random, one time it can be fine, next time it just glitches. Could it be the shadows/light source bugging out?

I just had it happen with the M-9 Tempest, so it's definitely not just the heavy pistols (which is what I thought it was only happening with for me before). Here's a screenshot:
