Proving area is probably easy to miss. As is making sure you visit every shop and the various houses within Redcliffe.Haasth wrote...

I don't get it , I've gone through 3 playthroughs gone everywhere including areas like from the thief quests and still no?
Did you ever enter the Proving Area?

Houses count as areas?
Worth a shot, at least. In all honesty, it's very difficult to help out with this seeing there are many areas. Perhaps random events even count (Seeing many random events reuse the same location).
When I unlocked it, it was totally random...

Berserker's Final Blow
Nightmare Combo
Entropic Death Combo
Mana Clash (can theoretically break 2,000)
You will get the "Explored All Areas" achievement if you:
1. Went to all of the main and side quest areas on the World Map, Deep Road Map and Denerim City Map
2. Went into every building
3. Went to jail
4. Found Gaxkang and any other "secret" areas

You can accomplish over 250 dmg with a spell (over time or considering dmg done to multiple targets) but not really with a single target spell like lightning or anything
did it with my mage, guessing it was on a aoe, TBH didnt really notice i had done it but its there in my achievement list. guessing it might be do able with a 2h zerker and buffs, i have been getting some big numbers with the death blow ability that adds your remaining stam into the damage.

I'm still trying to get Traveler and I'm not sure what area I've missed so far but this time I'm making sure to do all the sidequests to see if that opens something I've missed.

As for 250 dmg, death hex + death cloud does indeed rule. Morrigan managed to 446 damage with it at lvl 22 or so...

Gaxkang? How do I go to jail?
He's the boss of the Unbound quest line - you find a piece of paper in the Temple of Andraste and talk to a dwarf in Orzammar, then find him in a room in the back allies of Denerim.
You go to jail by yielding or losing to Ser Cautherine when you exit Arl Howe's mansion with Anora in Denerim.

Don't know how and don't see a way to look it up specifically but if you can "unlock" this on a party member just jump on a mage party member and pop one of the fancy spell combo things, should get it easily I would imagine.
***EDIT***
Before I forget, regarding the "Traveller" one, got that but game says I only explored 95% of the world...?

I somehow ended the game with 96% areas traveled. I did everything. Except that Slim in Denerim wouldn't give me a few of his quests, though I had both stealing and stealth. If he sends you to a new area in even one of those quests..I'm guessing that's what I missed.
It's not. I got the achievement without ever speaking to him. Most likely you missed the area where you get sent to kill the Kadan-Fe mercenaries, or perhaps the fort where you get imprisoned when you surrender, or the blood mage hideout - those are easy to miss, I think.

I got the "Heavy Hitter" thingy on my first playthrough, character was a stuntie S/S tank but for all I know I was controlling any other party member when it popped, I tend to view Achievements as sycophantic spam.

Did you skip Shale? DLC areas don't count for the achievement, while they are part of the percentage...

As for the 250 dmg hit, my Virulent Walking Bomb usualy did more than that. When the victim explodes it pretty much kills everything around it for me. This spell rocks.
