What button (360) is it when you grab a guy out of cover and insta-kill him? Haven't been able to get this to work, although I have seen it done on rare occasions.

Use melee when the fist icon pops up. Also, you get a medal for doing it 1, 3, and 5 times.

It is only possible to do by pressing the B (heavy melee) button while in a thin piece of cover with an enemy nearby/hiding behind it. Remember, you have to be in the cover, not just close to it, and hurry, because when you get too close, enemies will pull away from your cover to prevent you doing it. Be fast and just try it on individual targets at a time, going for broke will only get you broke.

You only need to tap it. I don't mean to split hairs but I find it becomes significant when the fist prompt doesn't always seem to show up as fast as it should. There's been a few times where I've had no fist prompt and manage to grab someone who went into cover for a split second just by tapping to make sure.

NEVER hold the button. You only need to see the fist icon [ very important ] and press once. It might take some time but tap it several times in a short burst, but never hold it or else you perform the winded up version of it!
