1. I don't blindly run into chaos (aka I don't play krogans or Vanguards) so turrets don't surprise me.
2. I know when Phantoms are supposed to show up so I'm constantly looking around for them.
Assault troopers seem to show up while I'm dealing with other problems and take me out...

Assault Troopers are to my knowledge the only NPC capable of "executing" players, so those saying AT's for that reason are misinterpreting the question. The other reasons stated for their inclusion on the list are valid though.

I'm still stickign with assault. Even without the execution, they are the biggest reason I die so much. When they start to crowd in, and force you to move from your spot they will either focus fire you down (or at least remove shields) while corraling you to other enemis, like the aformentioned turret.

This seems to be true. Just a few hours ago an Atlas sneaked up on me (i know how it sounds), blew me to hell and back and the kept pounding me over and over again but didn't execute me.Ultenth wrote...

OT I'd say Turrets and opportunistic Assault Troopers. I only got killed by a Phantom twice as I play Infiltrator and can get away easy and Asari Adept which laughs at Phantoms. Always prioritize them too.

The Phantoms... eugh. Overload and Energy Drain take out their barriers and affect them when the barrier is down so you can take them down.
That's if I notice one approaching instead of having one come up behind me while I was dealing with another threat. On Silver I've managed to hit one Phantom and have another right there.
The lone assault trooper who went unnoticed has also killed me a few times.

As a Vanguard, I really have trouble dealing with groups of Guardians when I don't get help. Those shielded sons of b*tches just won't go down!
Phantoms are easy for a Vanguard like me. I just charge and nova the crap out of them.

I die mostly because of improper thinking like enemies coming from all sides 2 guardian from north and one atlas and one phantom from south. I could kill phantom or atlas or run.. I die at the moment. I think way too much and darn im bad at running so i die there too...
Mostly i die from soldier attacking me at once...
Turrets are super-easy for me..
In brief: get cornered or too many soldiers....
