I think it's logical to assume there will be various outcomes varying on decisions made across all three games. One of the biggest features of the series is choice, or atleast the illusion of choice.

ME3 can provide closure by either ending the life of the Protagonist, or the main Antagonist, and thus it is not totally necessary for Shepard to die to end his series.

I Haven't seen this anywhere yet: Shepard and the galaxy fight valiantly, but still fall to the reapers; but not before securing some kind of asset for the next cycle's galactic inhabitants. Thus setting the stage for the next arc of the series.

It was confirmed in a article of the "Edmonton sun" that there will be "6 key endings", as Casey Hudson said in an interview with them. This could mean that Shepard may die in one or two of the endings, maybe even one where the reapers win.
Note however, that he said "key endings". This could mean, that depending on the players choices in the game, one key ending may have different varieties of its appearance.

I assume in some endings he lives, others he may die. But just because this particular trilogy in the Mass Effect universe is coming to a close doesn't mean Shepard has to die. It's a big universe, the Mass Effect one, with plenty of interesting characters. BioWare can always shift focus to another or other characters, perhaps you'll be an Asari Justicar who bumps into Shepard one day in the future (hence why we are told to keep our saves post ME3).

From this then I guess there are 6 differnt ending with veriations in the depending on choices.
I expect there will be an ending where the misson a complete success (maybe with all the reapers being destoryed) and one a complete failure where the Reapers kill everyone.
As well as a partial success ending. <- with these options that 3 ending.
Also I guess there will minior veriations like X appearing in ending or not.
Semi-major like if you made choice X you get ending parts A or B if you made choice Y you get ending parts C or D as part of your ending.
