I never experienced this problem in ME2. Bought SE on day 1. Eventually sold it for a used CE. 1st play-through was in first month of release. NG+ started last year and finished just 2 weeks ago. I even changed my Gamertag once since first purchasing ME2. Yet I still never saw this error. Never in the ME3 demo either. I did not experience this error when I first installed and started ME3 shortly after midnight launch.

However, after seeing that I could only import my NG+ ME2 details but not my face, I loaded ME2 to get my face code. Now I get this error in ME2 for the first time

My NG+ Shepard does not have a facecode, but my 1st Shepard does. So I wrote down the facecode and went back to ME3.

Still do not get this error in ME3.

Input face code after importing NG+ Shepard. Face is totally wrong. Being completely discouraged and not wanting to recreate my Shepard from scratch, I decide to quit ME3, watch some Doctor Who on Netflix, and eat. After eating I come back to ME3 to finally recreate my Shepard.

NOW I get this error in ME3!

What causes this error? No one else ever signed into my Xbox. I didn't move any files. I do not understand what I did to get this error.

This seems fixed for me in Mass Effect 3. I've not had a single problem since early yesterday.

Edit: Nope, still affected, but seems less common. On another note, now very angry that Bioware lied about MP being unneccesary for the best ending. On to MP i go...

Yep, this error is happening to me as well in Mass Effect 3. It's starting to get really irritating.

 Awsome, another great Mass Effect feature. Thought this was my exclusive one... but everyone else gets it too... Thought I would be someone special for you Bioware... 

How is it possible to create a game, a story, a triology like this and add such great features to it to destoy it that way? Bugs are the features from tomorrow.

I get the "you last played this on a different console" message a lot during ME2, once so far during ME3.
