Omni blade kill from behind cover?

When you and an enemy are on opposite sides of cover, a fist icon appears. I'm guessing that's supposed to activate the omni blade execution where you grab them and throw them to the ground.
Question is: How do you do it? Is it forward and space like if you were to jump out of cover?

I hit on this problem not long ago when instead of performing the omni-tool attack, my soldier would do a roundhouse swing with his Assault rifle when heavy melee was performed. This continued until the entire coop challenge was completed.
Does anyone know what might have caused this or why it happens? It certainly doesn't seem like an effective attack and it's not cool.

While we're at it, can we answer another question.

Yeah this is a weird bug that's occured to me a few times in MP. My Krogan soldier will for example stop headbutting and start rifle butting. Usually seems to just fix itself.

I would be up for it being an alternative. I already try to combine light melee and heavy melee effectively to stimulate close quarters (the only enemy I can't melee on Silver is Guardian, because they just turn around too fast. Even Atlas's and Phantoms are easier to at least finish off with a melee, although very difficult.)
Two light melees to stun, followed by a heavy kill shot, is very effective against isolated opponents.
Meleeing guardians is generally a bad idea. I only do it as a vanguard when charge fails to stagger them and I'm trying to buy time to charge again. Oh, and tac cloak, then heavy-meleeing both guardians before they can turn around is quite satsifying.

As for who's looks the coolest, nothing else even touches the Krogan cover kill xD Pull em over and ram your hand through their skull. Bahaha.
